Approve and authorize the Health Services Director, or his designee (Milt
Camhi) to execute on behalf of the County, Contract #27--480-1 with K.H.
Connie Wang-Chen, O.D. , a self-employed individual, in an amount not to
exceed $50, 000, for the period from August 1, 2002 through July 31, 2003, for
provision of professional optician services for Contra Costa Health .Plan
members .
FISCAL IMPACT:
This Contract is funded. by Contra Costa Health Plan member premiums . Costs
depend upon utilization. As appropriate, patients and/or third party payors
will be billed for services .
BACKOROUNDIREASON(S) FOR RECOMMENDATION(S) :
The Contra Costa Health Plan (Health Plan) has been obligated to provide
professional optician services, including eyewear, for Health Plan patients
with optician services as a covered benefit .
Approval of Contract #27-480-1 will allow the Contractor to provide services,
through July 31, 2003 .

Approve and authorize the Health Services Director, or his designee (Donna Wigand) to execute on
behalf of the County, Contract Amendment Agreement #24-933-15 with Crestwood Behavioral
Health, Inc., a corporation, effective June 1, 2002, to amend Novation Contract #24-933-13 (as
amended by Contract Amendment Agreement #24-933-14) to increase the payment limit by
$162,000, from$5,760,704 to a new total Contract Payment Limit of$5,922,704 and to increase the
automatic extension amount by$81,000,from$2,880,352 to a new total of$2,961,352.
FISCAL IMPACT:
This Contract is funded by Mental Health Realignment.

4n February 5, 2002, the Board of Supervisors approved Novation Contract #24-x933-13 (as
amended by Contract Amendment Agreement #24-933-14) with Crestwood Behavioral Health,
Inc., for the period from July 1, 2001 through June 30, 2002, which included a six-month
automatic extension through December 31, 2002, for the provision of sub-acute skilled nursing
care for County's Seriously and Persistently Mentally Ill (SMPI) and Neurobehavioral clients.
Approval of Contract Amendment Agreement #24---933--15 will allow the Contractor to provide
services to additional clients through December 31, 2002.

Psychiatric Transitions, a Corporation, effective May 1, 2002, to
amend Contract #74-143 , with no change in the original Contract
Payment Limit of $58,400 .
FISCAL IMPACT:
This Contract is 100° funded by Mental Health Realignment .
BACKGROUND/REASON t S FOR RECOMMENDATION(S) :
This Contract meets the social needs of County' s population in that
it provides a multi-disciplinary treatment program as an alternative
to hospitalization for adults who need active psychiatric treatment
for County' s mental health clients .
On October 16, 2001, the Board of Supervisors approved Contract #74-
143 with C.P.T. Inc. , (dba California Psychiatric Transition) , for
the period from October 1, 2001 through September 30, 2002, to
provide residential care and mental health services, medication
support, individual and group therapy services to County-referred
adults at Contractor' s treatment facility.
Approval of Contract Amendment Agreement #74-143-1 will modify the
rates in the Payment of the Contract, to allow Contractor to provide
a higher level of care services to County referred adults who need
additional specialized rehabilitative services, through September
30, 2002 .

SUBJECT: Request for Retroactive Contract#74-143-1
California Psychiatric Transitions
STATEMENT OF NEED FOR RETROACTIVE CONTRACT:
California Psychiatric Transitions (CPT) provides residential and mental health treatment
services to severely mentally ill Contra Costa adults. The current contract allows for
reimbursement at the rate of$160 per day, per client. This request for a retroactive contract does
not increase the overall payment limit, it increases the day rate at which we can reimburse CPT
for services. This rate change is pursuant to a clinical finding that two individuals recently
placed at CPT require a higher level of care and are receiving additional specialized
rehabilitative services. These services warrant a higher rate of reimbursement, and approval of
this contract will allow the Division to maintain these two clients at this level of care, rather than
a more costly higher levet of care.
BENEFIT TO CONTRA COSTA COUNTY:
Approval of this retroactive contract will allow the Division to maintain these two Contra Costa
County residents in the least restrictive level of care possible.
AUTHORIZATION OF SERVICES:
These services were requested/authorized by the Adult Program Chief, who is granted this
authority by the Mental Health Director, pursuant to the California Welfare and Institutions
Code, under the Bronzon-McCorquodale Act,W&I Code, Section 5608.

1 with Center on Deafness, a Non-profit Corporation, for the period
from July 1, 2002 through June 30, 2003 , in an amount not to exceed
$30, 952 , to provide mandated Intensive Day Treatment services for
a Contra Costa County resident minor in the AB 3632 Program who is
both deaf and seriously emotionally disturbed.

BACKGROUNOIREASON(S) FOR RECOMMENDATION(S) :
In January 2002, the County Administrator approved and the
Purchasing Services Manager executed Contract #74-146 with Center
on Deafness, for the period from May 15, 2001 through June 30, 2002
to provide an Intensive Day Treatment Program, Mental Health
services, and case management services for mentally or emotionally
disturbed deaf children, including Special. Education Pupils (SEP)
for Individualized Education Plan (IEP) required services under
AD3632 .
Approval of Contract #74-146-1 will allow the Contractor to continue
providing services through June 30, 2003 .

Children' s Services, Inc, , a non profit organization, in an amount not to
exceed $131, 286, for the period from July 1, 2002 through June 30, 2003 , for
the operation of a day treatment program for Seriously Emotionally Disturbed
(SED) adolescents and latency-age children at Milhous Treatment Ranch.
FISCAL IMPACT:
This Contract is funded 29% by Federal Medi-Cal, 61% by State EPSDT, and 10%
Employment and Human Services Department (EHSD) . Contractor will bill Medi-
Cal through the current County system under the Rehabilitation Option for
all eligible .services, and the Federal Financial Participation will accrue
to the County-
BACKGROUND/REASON(S) FOR RECCMMNNPATI0N(S) :
This Contract meets the social needs of County' s population in that it

need for hospitalization.
On August 7, 2001, the Board of Supervisors approved Contract #24-773-5
with Milhous Children' s Services, Inc . , for the period from July 1, 2001
through June 30, 2002, for the operation of a day treatment program for
Seriously Emotionally Disturbed (SED) adolescents and latency-age children
at Milhous Treatment Ranch.
Approval of Contract #24773-6 will allow the Contractor to continue to
provide services through June 30, 2003 .
