SPECiFiC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION RECOMMENDED ACTION Approve and authorize the Health Services Director, or his designee, (Jeff Smith, M.D. ) to execute on behalf of the County, Contract #26- 935-6 with David Kleinerman, M.D. , a self-employed individual, in an amount not to exceed $727,440, for the provision of professional urology services, for the period from August 1, 2002 through July 31, 2005 . FINANCIAL IMPACT: Cost to the County depends upon utilization. As appropriate, patients and/or third party payors will be billed for services . REASONS' FOR RECOMMENDATIONS/BACKGROUND: For a number of years the County has contracted with Medical and Dental Specialists to provide specialized professional services which are not otherwise available in its hospital and health centers. On August 17, 1999, the Board of Supervisors approved Contract #26-935- 4 (as amended by Administrative Amendment #26-935-5) with David Kleinerman, M.D. , for the period. from August 1, 1999 through July 31, 2002, for the provision of professional urology services, including but not limited to on-call coverage, teaching, and medical and/or surgical procedures . .Approval of Contract #26-935-6 will allow Dr. Kleinerman to continue providing services through July 31, 2005 . Walker, MD Health Services Director Costa DATE: August 13, 2002 --------- County SUBJECT: Approval of Contract for Consultant Services SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION RECOMMENDATION: Approve and authorize the Health Services Director, or his designee (Art Lathrop) to execute on behalf of the County, Consultant Services Agreement #23-333 with Fitch & Associates, LLC, effective September 1, 2002 through August 31, 2003, payment limit for this contract not to exceed $164,200. BACKGROUND: On November 3, 1998, the, the Board of Supervisors approved recommendation from the County Administrator, the Health Services Director, and the Contra Costa Fire Chief providing for modification of the County's emergency ambulance agreement with American Medical Response in order to help fund expansion of the Contra Costa Fire paramedic engine program. At that time the Board also directed Health Services and Contra Costa Fire to continue "working with AMR and with other fire agencies within the county to determine the feasibility of further integration of public and private paramedic services, conditioned upon the Identification of new or existing funding sources." In 1999, a committee of the Contra Costa Fire Chiefs' Association was formed to assist in identifying implementation steps and costs for countywide paramedic engine programs. Health Services EMS staff have participated in that committee and have met with AMR to discuss system changes which might support expansion of paramedic engine services. While there has been limited success in expanding paramedic engine services, to date fewer that half of the 66 fire stations in the county are paramedic-staffed, and despite a high degree of interest, several fire agencies have not begun to implement paramedic level service. While we have explored with AMR changes in EMS response configurations that would enable the County to shift existing ambulance subsidy to support fire paramedic services, discussion with AMR and with the fire services have not been successful in identifying sufficient funding to undertake implementation of countywide paramedic engine services. Proposals responding to the RFP were received from five firms. These proposals were reviewed by a selection committee comprised of two EMCC representatives, two County Fire Chiefs' Association representatives, a paramedic base hospital nurse coordinator, and the EMS Director. Based upon their proposal and experience in providing similar consulting services, Fitch & Associates was recommended by the selection committee. The Fitch & Associates contract provides for consulting services to be provided in three phases. Phase One ($91,2130) will include the system design and preparation of an ambulance service RFP and will be carried out in close consultation with each of the fire services. Phases Two and Three will include consultation to the EMS Agency in ambulance provider selection and contract negotiation, respectively. These phases will only be provided at the County's option following Phase One. FINANCIAL IMPACT: No impact on the county general fund. Walker, MD Health Services Director °`� �` Costa , DATE: August 13, 2002 con County SUBJECT: Approval of American Medical Response Contract Extension SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION RECOMMENDATION: Approve and authorize the Health Services Director, or his designee (Art Lathrop) to execute on behalf of the County, Ambulance Service Agreement #23-024-20 with Regional Ambulance, dba American Medical Response (AMR) to extend AMR's contract with County effective November 3, 2002 through October 31, 2003. Payment limit for this contract not to exceed $1,436,664 for emergency ambulance services and $237,312 per unit for up to two first responder paramedic units. BACKGROUND: To allow time for the development of a plan for integrating fire paramedic first responder services in those areas of the county where emergency ambulance services are currently provided by private ambulance, and, to carry out an emergency ambulance Request for Proposal process. The proposed AMR contract extension reflects an increase in allowable patient charges authorized July 1, 2002 pursuant to the existing contract to offset decreased Medicare collections resulting from implementation of the Medicare Fee Schedule April 1, 2002. Lowered County ambulance service subsidy of $119,722 per month compared to the previous subsidy level of$159, 435 per month, negotiated beginning June 1, 2002, pursuant to the existing contract for prior increases in Medicare payment rates. FINANCIAL IMPACT: No impact on the county general fund. Costs for contract will be charged to CSA EM-1 (Measure H). Approval of Contract #26-938-6, will allow Contractor to continue to provide General Surgery services for Contra Costa Regional Medical Center and Contra Costa Health Centers, through August 31., 2005 . BACKGROUND/REASON(S) FOR REC©MMENDATI©N(S) : The State Department of Mental Health provides funding to the Department ' s Mental Health Division to provide services for certain patients returning to the community from the State Hospital system, pursuant to Section 1604 of the Penal Code and known as the Conditional Release Program (CONREP) . On November 6, 2001, the Board of Supervisors approved Contract #24-629-13 with Hugh R.
