SUBJECT: APPROVE and AUTHORIZE the Employment and Human Services Director, or designee to

SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDED ACTION:
APPROVE and AUTHORIZE the Employment and Human Services Director, or designee enter into an
agreement with the California State Department of Health Services, Office of Long Term Care, in the amount of
$50,000 to continue planning for Long Term Care Integration and to develop an administrative action plan

integrated system of long term care in Contra Costa County for seniors and adults with disabilities. (#29-093)
FINANCIAL IMPACT-
I Requires a 20% ($10,000) match, 100% County. .
RA .KGROUND1
In March 1999, the Board authorized establishing a Long Term Care Planning Task Force to prepare an initial
administrative action plan and authorized submission of a first-year planning grant application to the State
i Office of Long Term Care. The State approved the grant application and awarded $50,000 to the County for FY
1999/2000. The Board authorized the FY 2000/2001 application and EHSD received a second year funding,
May 2000, in the amount of$50,000.
.The Employment and Human Services and Health Services Departments made application for a third year
$50,000 Long Term Care Integration Planning Grant, as approved by the Board April 24, 2001. The Contra
Costa County Long Term Care Integration Pilot Project (LTCIPP) submitted a request for the third year
Planning Grant from the Office of Long Term Care and is requesting the authority to enter into an agreement
with the State to continue the LTCI planning process.
With the continued commitment of the County's Employment and Human Services Department (EHSD),-
Contra
EHSD);Contra Costa Health Services Department (HSD) and over 100 service providers, caregivers, medical
practitioners and consumers, this collaborative effort will move onto its final phases of planning in FY
2001/2002.
The focus of the third year's planning effort will be to explore approaches to overcome challenges and barriers,
which were identified through the planning process, and to make decisions which address these issues. In this
way we will build upon the foundation laid by the Design Teams and the Task Force and move toward
achieving full consortium at risk Long Term Care Integration (LTCI). While much of the work completed to
date has been conceptual in manner, the work to be completed in FY 2001/2002 will provide the specificity
required to develop an Administrative Action Plan for transition to the LTCI de lopment phase.
