GILBERT, DIRECTOR OF GENERAL SERVICES o _ '+ Costa DATE: August 7, 2001 r County SUBJECT: APPROVING THE FIFTH AMENDMENT TO THE CONSULTING SERVICES AGREEMENT FOR NEW FIRE STATION NO. 79, 1423 LILLIAN STREET, CROCKETT (WH734B) SPECIFIC REQUESTS OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION I. RECOMMENDATION A. APPROVE Fifth Amendment authorizing extra work under the Consulting Services Agreement dated October 19, 1999 with Don Dommer Associates, for architectural services for New Fire Station No. 79, 1423 Lillian Street, Crockett for the Crockett-Carquinez Fire Protection District. The Fifth Amendment increases the scope of services and increases the payment limit as modified by the Second, Third, and Fourth Amendments from $86,830.00 to $94,345.00, an increase of$7,515.00. The First Amendment added the classification of Draftsman. B. AUTHORIZE the Director of General Services to execute the Fifth Amendment to the consulting services agreement and to issue written authorizations for extra work, in addition to the authorization for the Fifth Amendment provided, that the cumulative total extra cost for such additional authorizations shall not exceed $10,000.00. I.I. FINANCIAL IMPACT The Crockett-Carquinez Fire Station No. 79 project is funded with $603,370.00 Hazardous Mitigation Grant Program funds (HMGP) through the California Office of Emergency Services (75% of costs) and $202,456.00 fire district funds (25%of costs). An additional $80,000.00 of project costs is funded by the Crockett Improvement Foundation. III. REASONS FOR RECOMMENDATIONS/BACKGROUND A. On October 19, 1999 the Board of Supervisors approved and authorized the Director of General Services to execute the consulting services agreement with the Consultant. The Consultant was selected by the Fire District to provide architectural services for demolition and replacement of Fire Station No. 79 in Crockett at interviews held on July 26, 1999. Effective Date and Parties: Effective August 7, 2001, Don Dommer Associates (herein called "Consultant"), a California Corporation, and the County of Contra Costa(herein called"County"),a political subdivision of the State of California,mutually agree as follows: 2. Purpose: On October 19, 1999,the parties entered into a contract entitled"Consulting Services Agreement,"referred to as the"Agreement," which covers architectural services for the demolition and replacement of Fire Station No. 79 in Crockett for the Crockett-Carquinez Fire Protection District. The Agreement has previously been amended as follows: First Amendment dated December 8, 1999, Second Amendment dated May 8, 2000,Third Amendment dated October 9,2000,and Fourth Amendment dated February 6,2001. The parties desire to further amend the Agreement to expand the scope of service and to increase the payment limit accordingly. 3. Amendments to Agreement: A. In the Agreement,Section 1 (c),change the Payment Limit from$86,830.00 to$94,345.00,an increase of$7,515.00. B. In accordance with Section 14 of the Agreement provide the following extra services: Provide additional construction administration services required due to project duration. 4. Effect: Subject to the revisions made by this and any prior amendments,the Agreement shall remain in full force and effect. 5.
