SPECIFIC REQUESTS)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECONIMBIDATION(S) :
Approve and authorize the Health Services Director or his designee (Mary Foran)
to execute on behalf of the County, Grant Agreement #29-803 with the California
Health Foundation & Trust (for its California Telehealth & Telemedicine Center) ,
in the amount of $449,997, for the period from April 1, 2000 through March 31,
2002, for the Center for Health/Partners for Health Telehealth Network Project.
FISCAL IMPACT:
Approval of this Grant Agreement will result in $449,997 from the California
Health Foundation & Trust for the Center for Health/Partners for Health Telehealth
Network Project. No County funds are required.
BACKGROUND/RZASON(S) FOR RECOMIKENDATION(S) :
The Center for Health/Partners for Health Telehealth Network Project builds on
existing Center for Health services and links with the Partners for Health
neighborhood service center, as well as other health care, education and community
sites.
Project objectives include creating and maintaining a community-designed web site,
and a network of computer/Internet sites, and identifying and training residents
to become proficient in accessing and applying health information from the
Internet.
The Grantor has requested to be held harmless and indemnified as part of their
standard for approval of the Grant Agreement with the County. Except for funding
this Project, California Health Foundation & Trust will have no direct involvement
in the implementation of this Project. Therefore, in the view of County Risk
Management this indemnification will create a minimal potential loss. The Health
Services Department desires to expedite approval of the Agreement as written.
Approval of this Grant Agreement will provide funding for a two-year project to
create and provide culturally competent on-line health information to the Center
for Health and Richmond Health Center service area population.
Three certified copies of the Board Order should be returned to the Contracts and
Grants Unit.

Approve of Submission of Funding Application #29-388-36 to the State Department of
Health Services for AIDS Program Funding
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)A BACKGROUND AND JUSTIFICATION
RECOMMENDED ACTION:
A. Approve submission of Funding Application #29-388-36 to the State
Department of Health Services in the amount of $1,369,267, for the period
from July 1, 2000 through June 30, 2001, to continue funding for the AIDS
Case Management Program and authorize the Health Services Director, or
his designee (Wendel Brunner, M.D.) , to sign the necessary application
documents on behalf of the County; and
B. Authorize the Health Services Director, or his designee, to execute on
behalf of the County, any subsequent Standard Agreements) and technical
adjustments thereto, and any Memorandum of Understanding for the purposes
of securing grant funds and to implement and carry out the purposes
specified in the grant application.
FISCAL IMPACT:
Approval of the application for this project would result in $1, 369,267, from
the State Office of AIDS for the AIDS Case Management component of the Master
Grant Agreement, for the period from July 1, 2000 through June 30, 2001. No
County funds are required.
BACKGROUND/REASON(S) FOR RECOMMENDATION(S) :
The Master Agreement incorporates a Memorandum of Understanding (MOU) for each
service component of the AIDS program which defines the services to be provided
and the budget. The MOUS are negotiated by staff of the State Office of AIDS
and the County's AIDS Program representatives. The State requires only that
the MOUS be signed by the State Office of AIDS Chief and the County's Health
Officer, or his designee. This streamlines and expedites the contracting
procedure for the AIDS Program, as only the Master Grant Agreement and any
formal amendments to the Master Grant Agreement require County Board of Super-
visors and State Department of Finance approval.
The goal of the program, in operation since 1987, is to maintain client self-
sufficiency and to minimize costly hospitalization through the provision of
comprehensive case management services by a multi-disciplinary team consisting
of public health nurses and medical social workers and contracted support such
as home health care, nutritional counseling and home delivered meals.
In order to meet the deadline for submission, the application has been
forwarded to the State, but subject to Board approval. Three certified copies
of the Board Order authorizing submission of the application should be returned
to the Contracts and Grants Unit.

RECOMMENDED ACTION:
Approve and authorize the Health Services Director, or his designee,
(Frank Puglisi, Jr. ) , to execute on behalf of the County, Standard
Agreement #29-659 (State #00-20525) with the State Office of Statewide
Health Planning and Development, in the amount of $34,410, for the
period from July 1, 2000 through June 30, 2001, for the provision of
a health care team for Family Practice Residency Program.
F I S CAL IMPACT:
Approval of this Agreement will result in $34, 410 funding from the
State through June 30, 2001, for the Family Practice Residency
Program. No County funds are required.
BACKGROUND/REASON(S) FOR RECOMMENDATION{8) :
Under this Standard Agreement, the Family Practice Residency Program
at the Contra Costa Regional Medical Center, will recruit and employ
a Family Practice Resident, a primary care Pediatrician, and support
staff to create a health care team (the Sunshine Team) , to focus on
primary care delivery to extremely high-risk children and families in
the Concord area of Contra Costa County.
The primary purpose of the Sunshine Team is to identify, locate, and
contact those families in the target area that have children with
primary care needs . The team will utilize contacts with schools, day
care providers, Public Health Nurses, Headstart, and Juvenile services
to accomplish their goals.
Four certified and sealed copies of this Board Order should be
returned to the Contracts and Grants Unit for submission to the State.

The State reimburses Area Agencies on Aging for One-Time-Only costs incurred related to elections for the
California Senior Legislature (CSL). Funding for CSL elections comes from the California Fund for Senior
Citizens.

and Children's Services of the East Bay (#21-564-02) in the amount of$25,148 for services to
older refugees for the period of October 1, 2000 through September 30, 2001.

RECOMMENDED ACTION:
APPROVE and AUTHORIZE the Director of Employment and Human Services, or designee, to ACCEPT

BACKGROUND:
In June, 1999 Jewish and Family Children's Services of the East Bay in collaboration with the Lao Senior
Association and Catholic Charities of the East Bay applied for a federal Discretionary Grant to provide
bilinguanicultural services for refugees over 60 years of age who reside in Contra Costa County. Their
proposal was selected through a competitive review process at the federal level and was assigned to the County
for contract administration and oversight.
The Program has been in operation since October 1999 providing a variety of services for approximately 100
senior refugees who are Laotian, Vietnamese, Russian and/or Afghan. The program has achieved 100%of the
first year goals. Due to the outstanding program performance, a second year of funding has been awarded.
The Employment and Human Services Department may claim up to 5%of the 2000/2001 Discretionary Grant to
cover administration costs.
