SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDED ACTION:
Approve and authorize the Health Services Director, or his designee
(Donna Wigand) , to execute on behalf of the County, Contract #24- 939-8 (2)
with John Rostkowski, M.D. , in the amount of $50, 000, for the period from
July 1, 2000 through June 30, 2001, to provide Medi-Cal mental health
specialty services.
FISCAL IMPACT:
This Contract is funded by State and Federal FFP Medi-Cal Funds .
BACKGROUNDZREASON(S) FOR RECOMMENDATIONS:
On January 14 , 1997, the Board of Supervisors adopted Resolution 497/17,
authorizing the Health Services Department to contract with the State
Department of Mental Health to assume responsibility for Medi-Cal mental
health specialty services . Responsibility for outpatient mental health
specialty services involves contracts with individual, group and
organizational providers to deliver these services .
On July 20, 1999, the Board of Supervisors approved Contract #24-939-8 (1)
with John Rostkowski, M.D. , for the period from July 1, 1999 through June
30, 2000, for the provision of Medi-Cal mental health specialty services .
Approval of Contract #24-939-8 (2) will allow the Contractor to continue
to provide mental health specialty services, through June 30, 2001 .

SUBJECT: Approval of Novation Contract #74-044-1 with Lincoln Child Center
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)3 BACKGROUND AND JUSTIFICATION
RECOMBEMATION(S) :
Approve and authorize the Health Services Director or his designee (Donna Wigand) to execute
on behalf of the County, Contract #74-044-1 with Lincoln Child Center, for the period from July
1, 2000 through June 30, 2001, in the amount of $300,000, for the provision of Therapeutic
Behavioral services (TBS) to County referred clients ages 0-21 years of age.
FISCAL IMPACT:
This Contract is funded as follows:
$154,500 51.5* Federal FFP Medic-Cal Funds
145,500 48.5W State EPSDT funds
$300,000 Total Contract Payment Limit
CHILDREN'S IMPACT STATEMENT:
This Contract supports the following Board of Supervisors community outcomes: 1) Children ready
for and succeeding in school; 2) Families that are safe, stable, nurturing; and 3) Communities
that are safe and provide a high quality of life for children and families. The expected
outcomes include all goals identified by Children's Statewide System of Care guidelines
including: A) Increase and maintain school attendance as measured by school records; B) Increase
in positive social and emotional development as measured by the Child and Adolescent Functional
Assessment Scale (CAFAS) ; C) Increase in family satisfaction - as measured by the Parent
Satisfaction Survey; D) Decreased used of acute care system; and E) Placement at discharge to
a lower level of care.
BACKGROUND/RSASON(S) FOR RICOMMENDATION(S) :
The State Department of Mental Health has been working in collaboration with the County's Mental
Health Division to establish a mandated program to provide Therapeutic Behavioral Services (THS)
to children ages 0-21 years of age, who participate in the Early and Periodic, Diagnosis, and
Treatment (EPSDT) Specialty Mental Health services. This program will provide services to
eligible Medi-Cal beneficiaries under 21 years of age who meet the Mental Health Plan medical
necessity criteria, who are members of the certified class and who meet the criteria for needing
these services. On March 28, 2000, the Board of Supervisors approved Contract #74-044 with
Lincoln Child Center, for the period from March 1, 2000 through June 30, 2000 which included
a six-month automatic Contract extension through December 31, 2000.
Approval of Novation Contract #74-044-1 replaces the six-month automatic extension under the
prior Contract and allows Contractor to continue providing Therapeutic Behavioral services
(TBS) , through June 30, 2001.

RECOMMENDATION(S) :
Approve and authorize the Health Services Director, or his designee
(Donna Wigand) , to execute on behalf of the County, Contract #24-681-
31 (3) with Adelina Panlilio-Pilarski (dba The Sun Valley Residential
Care) , in the amount of $36, 000, for the period from July 1, 2000
through June 30, 2001, for the provision of augmented board and care
services for County-referred mentally disordered clients .
FISCAL IMPACT:
Funding for this Contract is included in the Department ' s Fiscal Year
2000-2001 Budget and is funded by Mental Health Realignment funding
100% .
BACKGROUND/REASON(S) FOR RECOMMENDATION(S) :
This Contract meets the social needs of the County' s population in
that it provides augmentation of room and board, and twenty-four hour
emergency residential care and supervision to eligible mentally
disordered clients, who are specifically referred by the Mental Health
Program Staff and who are served by County Mental Health Services .
In October 1999, the County Administrator approved and the Purchasing
Services manager executed Contract #24-681-31 (2) with Adelina
Panlilio-Pilarski (dba The Sun Valley Residential Care) , for the
period from December 1, 1999 through June 30, 2000, to provide
augmented board and care services for County--referred mentally
disordered. clients.
Approval of Contract #24-681-31 (3) will allow Contractor to continue
providing services through June 30, 2001 .

limit by $151, 125, from $781,946 to a new Contract payment limit
of $933, 071.

BACKGROUNDIREASON(S) FOR RECOMMENDATION(S) :
On July 27, 1999, the Board of Supervisors approved Contract
#25-010-5 with Center Point, Inc. for the period from July 1, 1999
through June 30, 2000, to provide shelter and support services for
homeless indigent individuals in Central County (Concord) and West
County (Richmond) for the County's Homeless Services Program, under
direction of the Health Services Department.
Approval of Contract Amendment Agreement #25-010-6 will allow
Contractor to provide services to additional homeless adults through
June 30, 2000.

execute on behalf of the County, Contract #24-925-4 with Lincoln Child Center in the amount
of $308,146, for the period from July 1, 2000 through June 30, 2001, for the provision of
school and community based mental health services for Medi-Cal eligible children and
families enrolled in the Early and Periodic Screening, Diagnosis, and Treatment (EPSDT)
Program.
FISCAL IMPACT:
This Contract is funded 52% by Federal FFP Medi-Cal Funds and 48% by State EPSDT Funds.
No County funds are required.
CHILDREN'S IMPACT STATEMENT:
This Mental Health EPSDT Program supports the following Board of Supervisors' community
outcomes: Children ready for and succeeding in school; Families that are safe, stable,
and nurturing; and Communities that are safe and provide a high quality of life for
children and families. Expected program outcomes include an increase in positive social
and emotional development as measured by the Child and Adolescent Functional Assessment
Scale (CAFAS) .
BACKGROUND/REASON(S) FOR RECOMMENDATION(S) :
This Contract meets the social needs of County's population in that it provides therapy,
medication support, case management, outreach, and crisis intervention services to
elementary aged students in East County in order to reduce the need for out-of-home
placements.
On August 3, 1999, the Board of Supervisors approved Contract #24-925-2 (as amended by
Contract Amendment Agreement #24-925-3) with Lincoln Child Center, for the period from July
1, 1999 through June 30, 2000, for the provision of school and community based mental
health services for Medi-Cal eligible children and families enrolled in the EPSDT Program.
Approval of this Contract 24-925-4 will allow the Contractor to continue providing services
through June 30, 2001.
