SPECIFIC REQUESTS)OR RECOMMENDATION(S)A BACKGROUND AND JUSTIFICATION
RECOMMENDATION(S) :
Approve and authorize the Health Services Director or his designee (Donna Wigand) to
execute on behalf of the County, Contract #74-070 with FamiliesFirst, Inc. , for the period
from July 1, 2000 through June 30, 2001, in the amount of $87,600, for operation of an
intensive day treatment program for Seriously Emotionally Disturbed (SED) adolescents.
FISCAL IMPACT:
This Contract is 100% funded by State EPSDT, SB90 funds, and Mental Health Realignment
(offset by Medi-Cal FFP)
Contractor will bill Medi-Cal through the current County system under the Rehabilitation
Option for all eligible services, and the Federal Financial Participation will accrue to
the County.
CHILDREN'S IMPACT STATEMENT:
This mental health day treatment program supports the following Board of Supervisors'
community outcomes: "Children Ready For and Succeeding in School"; "Families that are
Safe, Stable, and Nurturing"; and "Communities that are Safe and Provide a High Quality
of Life for Children and Families". Expected program outcomes include an increase in
positive social and emotional development as measured by the Child and Adolescent
Functional Assessment Scale (CAFAS) .
BACKGROUND/REASON(S) FOR RECOMMENDATIONS}
This Contract meets the social needs of County's population in that it provides mental
health day treatment for wards of the court to reduce the need for hospitalization.
Under Contract #74-070, Contractor will provide a day treatment program for Seriously
Emotionally Disturbed (SED) adolescents, including but not limited to rehabilitative day
treatment services under the Rehabilitation Option of Medi-Cal through June 30, 2001.

period from July 1, 2000 through June 30, 2001, for provision of professional
optician services .
FISCAL IMPACT:
This Contract is funded by Contra Costa Health Plan member premiums . Costs
depend upon utilization. As appropriate, patients and/or third party payors
will be billed for services .
BACKGROUND/REASON(S) FOR RECOMMENDATION(S) :
The Contra Costa Health Plan has been obligated to provide professional
optician services, including eyewear, for Health Plan patients with optician
services as a covered benefit .
On August 3 , 1999, the Board of Supervisors approved Non-Physician Services
Contract #27-376-1 with David Tam, O.D. , for the period from July 1, 1999
through June 30, 2000 .
Approval of Non--Physician Services Contract #27-3762 will allow the
Contractor to continue providing optician services to Contra Costa Health
Plan, through June 30, 2001 .

BACKGROUND/REASON(S) FOR RECOMMENDATION(S) :
The Health Plan has an obligation to provide certain specialized
professional health care services for its members under the terms of
their Individual and Group Health Plan membership contracts with the
County.
The Health Plan is also required under the terms of its Local
Initiative contract with the State, to contract with community
physicians and other providers, called "Safety Net" and "Traditional"
Providers, for the provision of medical care to Medi--Cal recipients .
Approval of this Contract #27-317--3 will allow the Contractor to
continue to provide podiatry services, through June 30 , 2001 .

RECOMMENDATION W:
Approve and authorize the Health Services Director, or his designee (Donna
M. Wigand) to execute on behalf of the County, (Novation) Contract
#24-681-42 (1) with J. A. P. C. (dba J.M.J. Retreat) for the period from July
1, 2000 through June 30, 2001, in the amount of $28, 800, for the provision
of augmented board and care services for County-referred mentally disordered
clients .
FISCAL IMPACT:
This Contract is 100% funded by Mental Health Realignment .
BACKGROTyND/REASON(S) FOR RECOMMENDATION(S) :
In May 2000, the County Administrator approved and the Purchasing Services
Manager executed Contract #24-681-42 with J. A. P. C. (dba J.M.J. Retreat) ,
for the period from February 15, 2000 through June 30, 2000 (which contained
a six-month automatic contract extension through December 31, 2000) , for the
provision of Augmented Residential Board and Care services .
The Department ' s residential support program, called Supported Adult Group
Enterprises (S .A.G.E. ) provides Augmented Board and Care Services designed
to meet the needs of the seriously persistently mentally ill (SPMI) adults,
who currently reside in locked facilities and state hospitals . J. A. P. C.
(dba J.M.J. Retreat) is one of the few board and care facilities that was
able to meet the S .A.G.E. Program requirements and provide permanent housing
with on-,site support that allows mental health consumers to fully integrate
into the larger community.
Approval of Novation Contract #24-681-42 (1) replaces the six-month automatic
extension under the prior Contract and allows Contractor to continue
providing services through June 30, 2001 .

Approve and authorize the Health Services Director, or his designee (Donna
Wigand) , to execute on behalf of the County, Contract #244939-49 (2) with
Catholic Charities of the East Bay, for the period from July 1, 2000 through
June 30, 2001, to provide Medi-Cal mental health specialty services, to be
paid as set forth below:
a. S .50 per minute for mental health services, as described in
the CCHMP Provider Manual, excluding group therapy sessions;
b. S .1A� per minute per individual for group therapy sessions, not
to exceed 90 minutes per session, as described in the CCHMP
Provider Manual ; and
C. S 1.00 per minute for medication support services, as described
in the CCHMP Provider Manual .
FISCAL IMPACT:
This Contract is funded by State and Federal FFP Medi-Cal Funds .
t ACKGROUND IREASON t S) FOR RECOMMENDAT ONS:
On January 14, 1997, the Board of Supervisors adopted Resolution #97/17,
authorizing the Health Services Director to contract with the State
Department of Mental Health to assume responsibility for Medi-Cal mental
health specialty services . Responsibility for outpatient mental health
specialty services involves contracts with individual, group and
organizational providers to deliver these services .
On July 20, 1999, the Board of Supervisors approved Contract #24-939--49 (1)
with Catholic Charities of the East Bay, for the period from July 1, 1999
through June 30, 2000, for provision of Medi-Cal mental health specialty
services .
Approval of Contract#24-939-49 (2) will allow the Contractor to continue
providing Medi-Cal mental health specialty services, through June 30, 2001 .
