Approve and authorize the Health Services Director, or his designee

#22-779 with Wendy Wright, for the period from August 1, 2000 through
July 31, 2001, in the amount of $52, 209, for the provision of data and
research analysis for Public Health' s Community Wellness & Prevention
Program (CW&PP) .
FISCAL IMPACT:
This Contract is included in the Health Services Department ' s Enter-
prise I budget and is funded 1000 by State and County Funds .
BACKGROUND/REASON(S) FOR RECOMMENDATIONS :
Under Contract #22-779, Wendy Wright will provide data and research
analysis services to the Public Health Department ' s Community Wellness
& Prevention Program with regard to Epidemiology. Contractor' s
services shall include, but are not limited to, writing reports,
conducting analysis, interpreting findings, and displaying results of
the methodologies, principles, and practices of Epidemiology through
July 31, 2001 .

SUBJECT: Approval of Amendment #26-162-15 with the Regents of the University of
California (U.C. Davis School of Internal Medicine Department)

RECOMMENDATION(S}_ :
Approve and authorize the Health Services Director, or his designee
(Frank Puglisi) , to execute on behalf of the County, Amendment #26-162-
15 (University 93-03684V) with the Regents of the University of
California (U. C. Davis School of Internal Medicine Department) , to
amend Contract #26-162-9, to increase the Contract Payment Limit by
$80,000 from $660, 000 to a new total of $740, 000, to provide additional
oncology/hematology services, through June 30, 2001 .
FISCAL IMPACT:
This Contract is funded in the Department ' s Enterprise I Budget . The
services provided under this agreement are billable to patients and
appropriate third party payors .
BACKGROUNDIREASON(S) FOR RECOMMENDATION(S) :
On July 13 , 1993, the Board of Supervisors approved Contract #26-162-9
(as amended by Amendment Agreements #26-162-10 through #26-162-14) with
the, Regents of the University of California (U.C. Davis School of
Internal Medicine Department) , for the period from July 1, 1993 through
June 30, 2000, for professional oncology services for patients treated
at Contra Costa Regional Medical Center and Contra Costa Health
Centers .
For several years, the Board of Supervisors has approved contracts with
the Regents of the University of California (U.C. Davis School of
Medicine) for professional services in several medical specialties.
Services are provided to County' s inpatients and outpatients at Contra
Costa Regional Medical Center and Contra Costa Health Centers by
physicians who are U.C. Davis School of Medicine faculty members.
Approval of Amendment #26-162-15 will allow Contractor to provide
additional Hematology/Oncology clinic services under this Agreement
through June 30, 2001 .

RECOMMENDATION(S) :
Approve and authorize the Health Services Director, or his designee
(Donna Wigand) , to execute on behalf of the County, Contract #74-065 with
Pathway School, in the amount of $30, 776, for the period from July 1,
2000 through June 30, 2001, for the provision of an Intensive Day
Treatment Program, mental health :services, medication support, and case
management services .
FISCAL IMPACT:
This Contract is 1005 State funded.
CHILDREN'S 1XPACT STATEMENT:
This Mental Health Program supports the following Board of Supervisors'
community outcomes : "Children Ready For and Succeeding in School" ;
"Families that are Safe, Stable, and Nurturing" ; and "Communities that
are Safe and Provide a High Quality of Life for Children and Families" .
Expected program outcomes include an increase in positive social and
emotional development as measured by the Child and Adolescent Functional
Assessment Scale (CAFAS) .
BACKGROUND/REASON(S) FOR RECOMMENDATION(S) :
This Contract meets the social needs of County' s population in that it
provides a multi-disciplinary treatment program as an alternative to
hospitalization for minors who need active psychiatric treatment for
acute mental, emotional or behavioral disorders .
Under Contract #74-065, Pathway School will provide mental health
services for mentally or emotionally disturbed children including Special
Education Pupils (SEP) for Individualized Education Plan (IEP) as
required under AB3632 , through June 30, 2001 .

RECOMMENDATION(8) :
Approve and authorize the Health Services Director, or his designee (Chuck
Deutschman) to execute on behalf of the County, Contract Amendment Agreement
#24-617-16 with Ujima Family Recovery Services, effective March 1, 2000, to
amend Contract #24-617-12 (as amended by Amendment Agreements #24-617-13,
#24-617-14, and #24-617-15) to increase the Payment Limit by $117,724 from
$1, 052, 097 to a new total Payment Limit of $1, 169,821.
FISCAL IMPACT:
This Contract is 75. 8% Federally funded, 14 . 1% State funded, and 10. 1%
County funded.
CHILDREN'S IMPACT STATEMENT:
This Alcohol and Drug Abuse program supports the Board of Supervisors,
"Families that are Safe, Stable, and Nurturing" community outcome by
providing parenting education, stability, and safety for mothers (and their
children) and pregnant women who are alcohol and drug dependent. Expected
outcomes include delivery of drug-free babies, decreased use of alcohol,
tobacco and other drugs, reduction in the number of relapses, and creation
of a sober social network.
BACKGROUNDLREASON(S) FOR RECOMMENDATION(S) :
On December 14, 1999, the Board of Supervisors approved Contract #24-617-12
(as amended by Amendment Agreements #24-617-13, #24-617-14 , and #24-617-15)
with Ujima Family Recovery Services, for the period from July 1, 1999
through June 30, 2000 (which included a six-month automatic extension
through December 31, 2000) for the provision of residential and outpatient
intensive day alcohol and drug abuse treatment services for women.
Approval of Contract Amendment Agreement #24-617-16 will allow the
Contractor to provide additional services for CalWORK.s mothers and their
children, through June 30, 2000.

SUBJECT: Retroactive Contract Amendment for Ujima Family Recovery Services
Contract#24-617
We request a retroactive contract amendment for Ujima Family Recovery Services for
FY 1999-2000 to increase the payment limit on contract#24-617 by an additional
$117,724. The background information for this necessary action is as follows:
A contract amendment to increase the payment limit on this contract by $117,724 was
approved by the Board of Supervisors effective February 1, 2000 (Ca1WORKS funds,
contract#24-617-13). Subsequently, we submitted another contract amendment to
increase the payment limit by an additional $158,088 in Ca1WORKS funds for
supplementary activities. This amendment was approved by the Board effective March 1,
2000 (Ca1WORKS funds, contract#24-617-14). These two amendments should have
increased Ujima's payment limit by a total of$275,812.
Inadvertently, instead of increasing Ujima's payment limit by an additional $158,088, the
contract was increased by only $40,364, to equal a total increase of$158,088 (and not the
required total of $275,812). Neither CSAS staff nor Ujima staff caught the error at the
time of the March amendment. The additional $117,724 in funds is requested at this time
to provide necessary payments to Ujima for completed services and activities.

Portia Bell Hume Behavioral Health and Training Center, for the period from
July 1, 2000 through June 30, 2001, to provide Medi-Cal mental health
specialty services, to be paid as set forth below:
a. S .50 per minute for mental health services, as described in
the CCHMP Provider Manual, excluding group therapy sessions;
b. S .14 per minute per individual for group therapy sessions, not
to exceed 90 minutes per session, as described in the CCHMP
Provider Manual; and
C . S 1.00 per minute for medication support services, as described
in the CCHMP Provider Manual .
FISCAL IMPACT:
This Contract is funded by State and Federal FFP Medi-Cal Funds .

On January 14, 1957, the Board of Supervisors adopted Resolution #97/17,
authorizing the Health Services Director to contract with the State
Department of Mental Health to assume responsibility for Medi- Cal mental
health specialty services. Responsibility for outpatient mental health
specialty services involves contracts with individual, group and
organizational providers to deliver these services .
On August 3, 1599, the Board of Supervisors approved Contract #24-949-67 (1)
with Portia Bell Hume Behavioral Health and Training Center, for the period
from July 1, 1999 through June 30, 2000, for provision of Medi-Cal mental
healthspecialty services .
Approval of Contract #24-949-67 (2) will allow the Contractor to continue
providing Medi-Cal mental health specialty services, through June 30, 2001.

Cost to the County depends upon utilization. As appropriate,

BACKGROUND/REASON(S) FOR RECOMMENDATION(S) :
For a number of years the County has contracted with Medical and
Dental Specialists to provide specialized professional services
which are not otherwise available in its hospital and health center.
On August 17, 1999, the Board of Supervisors approved Contract #26-
944-2 (as amended by subsequent Amendments #26-944-3 and #26-944-4) ,
with Shahin Javaheri, M.D. , for the period from August 1, 1999
through July 31, 2000, for the provision of Medical Specialty,
Plastic Surgery services .
Approval of Contract #26-944-5 will allow Dr. Javaheri to continue
providing services through July 31, 2003 .

a. For consultation, training, medical and/or surgical procedures,
Contractor shall be paid as follows :
(1) X14, 563 . 04 per month, for the period from ,August 1, 2000
through July 31., 2001, not to exceed a total of 5174.756;
(2) $15,460.00 per month, for the period from August 1, 2001
through July 31, 2002, not to exceed a total of $185,520;
(3) $16,356 .00 per month, for the period from August 1, 2002
through July 31, 2003, not to exceed a total of $196,272;
and
b. $1,200.00 per month, for on,-call coverage services, NOT To
EXCEED a total of $14,400; and
C . 5450.00 per occurrence, when Contractor is required to come to
the hospital during on-call hours for a surgical procedure.
In the event that Contractor works less than a full month at
Contra Costa Regional Medical Center and Contra Costa Health
Centers, County shall prorate payments to Contractor for that
