SPECIFIC REQUEST(S)OR RECOMMENDATION(S)6 BACKGROUND AND JUSTIFICATION
RECOMMZMED ACTYON:
A. Approve and authorize the Health Services Director or his designee (Donna Wigand) to
execute on behalf of the County, Life Support Residential Care Placement Agreement #24-
368-12 (5) with Carefilled Homes (dba Willow House) , for the period from July 1, 2000
through June 30, 2001, to provide residential care for mentally disordered offenders
under the County's Conditional Release Program (CONREP) .
B. Approve the following rates for specialized room, board, care and supervision provided
under this Agreement:
24.16 per day, per client for each 31 day calendar month;
24.97 per day, per client for each 30 day calendar month; and
25.83 per day, per client for each 29 day calendar month.
FINANCIAL IMPACT:
This Agreement is totally State-funded under County's Standard Agreement #29-441-21 with the
State Department of Mental Health for the CONREP Program. No County funds are required.
REASONS FOR RECOMMENDATIONS/BACKGROUND:
The County's Standard Agreement #29-441-21 with the State Department of Mental Health
provides for State funding of County Mental Health services for certain patients returning
to the community from the State Hospital system, pursuant to Section 1604 of the Penal Code.
This program, known as the Conditional Release Program, or CONREP, is totally State-funded
and allows the County to use a portion of these funds to pay the costs of specialized room,
board, care and supervision for certain program clients who might otherwise require some
other form of public assistance.
On September 21, 1999, the Board of Supervisors approved Contract #24-368-12 (4) with
Carefilled Homes (dba Willow House) , for the period from July 20, 1999 through June 30, 2000,
to provide residential care for mentally disordered offenders under the County's Conditional
Release Program (CONREP) . Approval of Contract #24-368-12 (5) will allow the Contractor to
continue to provide residential care services to CONREP clients, through June 30, 2001.

On January 4, 2000, the Board of Supervisors approved Contract
#74-029 with John Kady, for the provision of consultation and
technical assistance to the Department, with regard to vocational

period from December 16, 1999, through June 30, 2000.
Approval of Contract #74-029-1 will allow the Contractor to continue
providing services through June 30, 2001.

for the period from July 1, 2000 through June 30, 2001, for
psychiatric services to County patients at the Adult Mental Health
Clinic in Concord.
FISCAL IMPACT•
This Contract is funded by Mental Health Realignment funding,
off-set by Medi-Cal, Medicare, private insurance and patient fee
collections, and is included in the Department ' s Fiscal Year Budget .
BACKGROt=/REASON(S) FOR RECOMMENDATIONS
For a number of years the County has contracted with Medical, Dental
and Mental Health Specialists to provide specialized professional
services which are not otherwise available in its hospital and
health centers .
On December 1, 1999, the Board of Supervisors approved Contract #24-
998-1 with George Ponomareff, M.D. , to provide psychiatric services
to County patients at the Adult Mental Health Clinic in Concord, for
the period from December 1, 1999 through June 30, 2000 .
Approval of Contract #24-998-2 will allow Contractor to continue
providing services through June 30, 2001 .

Residential and Day Treatment Center, Inc. in the amount of $273 ,750, for
the period from July 1, 2000 through June 30, 2001, for the provision of
intensive day treatment for County-referred minors.
FISCAL IMPACT•
This Contract is funded 100% by Mental Health Realignment.
CHILDREN'S IMPACT STATEMENT:
This Mental Health Program supports the following Board of Supervisors'
community outcomes: "Children Ready For and Succeeding in School";
"Families that are Safe, Stable, and Nurturing" ; and "Communities that are
Safe and Provide a High Quality of Life for Children and Families" .
Expected program outcomes include an increase in positive social and
emotional development as measured by the Child and Adolescent Functional
Assessment Scale (CAFAS) .
BACKGROUNDIREASON(S) FOR RECOMMENDATION(S)_:
This Contract meets the social needs of County' s population in that it
provides a multi-disciplinary treatment program as an alternative to
hospitalization for minors who need active psychiatric treatment for acute
mental, emotional or behavioral disorders.
On August 17, 1999, the Board of Supervisors approved Novation Contract
#24-602-16 with Seneca Residential and Day Treatment Center, Inc. , for the
period from July 1, 1999 through June 30, 2000, for the provision of
intensive day treatment, mental health, medication support, crisis & case
management services for County-referred minors.
Approval of Contract #24-602-17 will allow the Contractor to continue
providing services through June 30, 2001.

Approve and authorize the Health Services Director or his designee (Donna
Wigand) to execute on behalf of the County, Contract #24-794-5 (4) with
Alta Bates Medical Center for its Herrick Hospital, for the period from
July 1, 1999 through June 30, 2000, for Fee-For-Service (FFS) Medi-Cal
acute psychiatric inpatient services, to be paid in accordance with the
following rate schedule:
$515. 00 For each Medi-Cal "Acute Psychiatric Day of Service",
$600.00 For each non-Medi-Cal "Acute Psychiatric Day of Service", and
$273 .35 For each "Administrative Day" .
FISCAL IMPACT•
This Contract is funded 100% by acute psychiatric inpatient consolidation
funds.
BACKGROUNDIREASON(S) FOR RECOMMENDATION(8I :
Chapter 633 , Statutes of 1994, AB 757, authorized the transfer of state
funding for Fee-For-Service/Medi-Cal (FFS/MC) acute psychiatric inpatient
hospital services from the Department of Health Services to the
Department of Mental Health (DMH) . On January 1, 1995, the DMH
transferred these funds and the responsibility for authorization and
funding of Medi-Cal acute psychiatric inpatient hospital services to
counties that chose to participate in this program.
On July 12, 1994, the Board of Supervisors notified the State of its
intent to function as the Local Mental Health Care Plan and to assume the
responsibility of inpatient consolidation. The State requires that
counties contract with those hospitals which are designated "safety net
providers" and certain hospitals designated as "traditional providers"
Therefore, the County is required to contract with Herrick Hospital (a
traditional provider. )
On October 27, 1998, the Board of Supervisors approved Contract
#24-794-5 (3) with Alta Bates Medical Center for its Herrick Hospital for
the period from July 1, 1998 through June 30, 1999 . Approval of Contract
#24-794-5 (4) will allow the Contractor to continue providing services
through June 30, 2000.

SUBJECT: Request for Retroactive Approval for Contract with Alta Bates
Medical Center (Herrick Hospital) #24-794-5(4)
Statement of Need for Retroactive Contract #24-794-5(4):
According to Medi-Cal consolidation regulations, the rates for a hospital are negotiated
by the county where the hospital is located, in this case Alameda County. Mental
Health Division staff experienced some difficulty in obtaining these rates in a timely
manner. The negotiations this year were evidently complicated, and thereby delayed,
by hospital reorganization issues.
Benefit to Contra Costa County:
This contract is necessary to provide inpatient psychiatric care to adults who are
seriously and persistently mental ill as an alternative to the State Hospital.
Authorization of Services:
These services were requested/authorized by the Crisis Services Program Chief, who is
granted this authority by the Mental Health Director, pursuant to the California Welfare
and Institutions Code, and under the Bronzon-McCorquodale Act, W&I Section 5608.

Approve and authorize the Health Services Director or his designee (Donna Wigand) , to
execute on behalf of the County, Contract #24-838-6 with Contra Costa Association for
Retarded Citizens, Inc. in the amount of $130,000, for the period from July 1, 2000 through
June 30, 2001, for the provision of Early and Periodic Screening, Diagnosis, and Treatment
(EPSDT) Program services at Lynn Center. This Contract includes a six-month automatic
extension through December 31, 2001, in the amount of $65,000.
FISCAL IMPACT:
This Contract is funded as follows:
$ 66,600 Federal FFP Medi-Cal Funds
63.400 State EPSDT Funds
$130,000 Total
CHILDREN'S IMPACT STATEMENT:
This EPSDT Program supports the following Board of Supervisors' community outcomes:
"Children Ready For and Succeeding in School" ; "Families that are Safe, Stable, and
Nurturing"; and "Communities that are Safe and Provide a High Quality of Life for Children
and Families". Expected program outcomes include an increase in positive social and

(CAFAS) ; an increase in family satisfaction as measured by the Parent Satisfaction Survey;
and a decrease in out-of-home placements.
BACKGROUNDZREASON(S) FOR RECOMMENDATION(S) :
This Contract meets the social needs of County's population in that it provides in-home,
transition, and outpatient services for Medi-Cal eligible children and their families
enrolled in Lynn Center's EPSDT program in order to keep these clients out of higher levels
of care, including State Hospitals.
On August 17, 1999, the Board of Supervisors approved Contract #24-838-5 with Contra Costa
Association for Retarded Citizens, Inc. , for the period from July 1, 1999 through June 30,
2000, (which included a six-month automatic extension through December 31, 2000) for the
provision of EPSDT Program services at Lynn Center. Approval of Novation Contract
#24-838-6 will replace the six-month automatic extension under the prior Contract, and
allow the Contractor to continue providing services through June 30, 2001.
