FISCAL IMPACT• Approval of this Agreement will result in a total payment to the County of $26, 000 . No County match is required. BACKGROUND/REASON(S) FOR RECOMMENDATION(S) : On June 15, 1999, the Board of Supervisors approved Interagency Agreement #29-485-5, with Contra Costa County Superintendent of Schools, for the period from July 1, 1999 through June 30, 2000, to provide professional mental health intervention services to County- designated severely emotionally disturbed Special Education students, who are participating in AB 1261. Classroom at the Marchus Center, and their families . Under this Interagency Agreement, #29-485-6, County Superintendent of Schools (office of Education) will continue to pay County for the provision of professional mental health intervention services through June 30, 2001 . Approve and authorize submission of Funding Application #29-455-10 to the State Department of Mental Health in the amount of $61,298, for FY 2000-2001, to continue PATH Formula Grant Funding under the Stewart B. McKinney Homeless Assistance Amendments Acts of 1990 (PL 101-645) . B. Authorize the Health Services Director, or his designee (Donna Wigand) to sign required assurances and certifications regarding debarment and suspension, a drug-free workplace, lobbying, and compliance with Federal requirements, and to make any minor technical adjustments to the Grant Application that may be required by the State Department of Mental Health. FISCAL IMPACT: Approval by the State Department of Mental Health of this application will result in $61,.298 of Federal PATH Grant Funds for the continuation of outreach services to the homeless mentally disabled. The application requires an additional non-Federal match ($21, 000) which will be provided through existing County Realignment funds. BACKGROUND/RNASON(S) FOR RNCOHMNDATION(S) : On July 27, 1999, the Board of Supervisors approved the Federal PATH Grant Application #29-455-9 for Fiscal Year 1999-00. Approval of this application #29-455-10 will continue the PATH project operated by Phoenix Program, Inc. , under County's Contract #24-385, for the provision of various outreach, treatment and case management services to persons who have a serious mental illness, or have a co-occurring serious illness and substance abuse disorder, and who are homeless or at imminent risk of becoming homeless. The Federal PATH Grant Application for 2000-2001 was reviewed and recommended for approval by the County's Mental Health Commission on May 25, 2000. In order to meet the deadline for submission, the application has been forwarded to the State, but subject to Board approval. Three certified copies of the Board Order authorizing submission of the application should be returned to the Contracts and Grants Unit for submission to the State. Approve and authorize the Health Services Director, or his designee (Donna M. Wigand, LCSW) to submit the Federal SAMHSA Grant Renewal Application #29-472-9 to the State Department of Mental Health, for $1,421,004 in Federal SAMHSA Grant Funds, for the period from July 1, 2000 through June 30, 2001, to continue Federal Grant-funded mental health services. B. Authorize the Health Services Director, or his designee (Donna M. Wigand, LCSW) to execute, on behalf of the County, the following application documents: Assurance of Compliance with Public Law 102-321; and Certifications (regarding lobbying, salary rate cap, drug free work environment; debarment, suspension ineligibility and voluntary exclusion; and environmental tobacco smoke) . C. Authorize the Health Services Director, or his designee (Donna M. Wigand, LCSW) to make minor changes in said Application, as may be requested by the State, to maintain compliance with State and Federal requirements. FISCAL IMPACT: The SAMHSA Grant Renewal Application describes service program changes which are necessary for the County to remain eligible to continue to receive this $1,421,004 Federal Grant Allocation, and presents a plan and detailed provider budgets for use of these funds. No County matching funds are required. REASON(S) FOR RECO>tiIl�TION(S)_: The FY 2000-2001 SAMHSA Grant Renewal Application was reviewed and recommended for approval by the County's Mental Health Commission at its meeting on June 22, 2000. The Federal "Substance Abuse and Mental Health Services Administration" (SAMHSA) Grant Program utilizes the Federal Grant funds for integrated systems of care to serve the target populations of adults and older adults who are seriously and persistently mentally ill and seriously emotionally disturbed (SED) children and youth. While approval of this document does not obligate the County to make any expenditures, submission and approval by ,the State are required in order for the County to receive these Federal Grant Funds from the State in FY 2000-2001. In order to meet the deadline for submission, the document has been forwarded to the State, but subject to Board approval. Diablo Community Health Benefit Corporation SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION RECOMMENDED ACTION: Approve and authorize the Health Services Director or his designee (Wendel Brunner, M.D. ) to accept on behalf of the County, a Grant Award from the John Muir/Mt . Diablo Community Health Benefit Corporation, in the amount of $35, 000 for the period April 1, 2000 through March 31, 2001 for the Breast Cancer Partnership Patient Navigation Project . FINANCIAL IMPACT: Acceptance of this Grant Award will result in $35, 000 from the John Muir/Mt . Diablo Community Health Benefit Corporation. No County funds are required. BACKGROUND/REASON(3) FOR RECOMMENDATIONS : Contra Costa County has the second highest rate of breast cancer in California. Through the Contra Costa County Breast Cancer Partnership (CCCBCP) , low income women over forty years old are receiving necessary mammograms. However, in instances where potential problems are detected in non-English speaking women, treatment services are difficult to identify. Acceptance of this Grant Award will allow the CCCBCP to assist non-English speaking women diagnosed with breast cancer to link to appropriate and timely medical and support services. Approval of Grant Award #28-610-3 will allow the County to continue the Breast Cancer Partnership Patient Navigator Project, through March 31, 2001 . Three certified copies of the Board Order should be returned to the Contracts and Grants Unit . FISCAL IMPACT: Approval of this agreement by the State Department of Health Services will result in $329,393, for 'Fiscal Year 2000-01, for continuation of the Immunization Assistance Program. No County funds are required. RBASOKS FOR RRCAHQ+WWATION'SIBACKGROUND: On May 16, 2000, the Board of Supervisors approved submission of Funding Application #29. 208-62, to the State Department of Health Services, for continuation of the long-standing Immunization Assistance Program. Standard Agreement #29-208-63 is the result of that application and provides funding to continue services through June 30, 2001. The County's Immunization Program makes immunizations available to all persons in need of such service in order to prevent the occurrence and transmission of childhood diseases; and under the terms of this Contract, the State provides free vaccine to the County. The Program monitors the compliance of preschools, elementary schools, and secondary schools in meeting State-mandated immunization requirements through inservice programs and limited technical assistance. An adverse reaction monitoring system and outbreak control team are also included in the Program. Four certified and sealed copies of this Board Order should be returned to the Contracts and Grants Unit for submission to the State.
