RECL>MMDE 3 ACTION-
A. Approve and authorize the Health Services Director or his designee (Donna
Wigand) to execute on :behalf of the County, Life Support Residential Care
Placement Agreement #24-368-12 (4) with Carefilled Homes (dba Willow House) for
the period from duly 20, 1-999 through June 30, 2000, to provide residential
care for mentally disordered offenders under the County's Conditional Release
Program {CONREP1 .
E. Approve the following rates for specialized room, board, care and supervision
provided under this Agreement:
:
23.58 per clay, per client for each 31 day calendar month
24.37 per dray, per client for each 30 day calendar month, and
25.21 per day, per client for each. 29 day calendar month..
FINANCIAL IMPACT:
This Agreement is totally State-fended under County's Standard Agreement ##29-441-20
with. the State Department of Dental Health for the CON'REP Program. No County funds>
are required.
The County's Standard. Agreement #29-441-20 with the State Department of 'Menta' Health
provider for State funding of Counrly Mental `health services for certain patients
returning to the community from: the State Hospital system, pursuant to Sectio or 1504
of the Penal Code
This program, known as the Conditional Release Program, or CQNREP, is to-ally State-
funded
tate-
fu ded and allows the County to use a portion of these funds to pay the costs of
specialized room, board, care and supervision for certain program; clients who right
otherwise require some other form of public assistance.
Approval of Contract ##24-368.12{4) will allow the Contractor �.o provide residential
care services to CON"R P clients, through June 30, 2000.

Approve and authorize the Health Services Director, or his designee
(Frank Fuglisi, Jr. ) , to execute on behalf of the County Contract #26-
009-21, with Alhambra Radiology Associates, for the; period from jury
_, 1999 through June 30, 2002, for the provision of professional
diagnostic imaging services, to be paid in accordance with the
attached fee schedule.
FINANCIALIMPACT:
This contract is included in the Health Services Department Enterprise
I budget . As appropriate, patients and/or third-party payors will be
billed for radiology services
ASC FOR RECOMMENDATIONS/BACKGROUND:
This Contractor has provided essential processional diagnostic imaging
services to the Department for many years. Due to the fact that the
Contractor has agreed to perform imaging services previously seat to
other facilities, County and Contractor have mutually agreed to
terminate the exisuing services under Contract #26-009-19 (as amended
by Contract Amendment Agreement #26-009-20 and to substitute Contract
#26-009-21 in its place .
Under Contract #26-009-21 Contractor will provide diagnostic imaging
services through June 30, 2002 .

Approve and authorize the Health Services Director or his designee (Donna Wigand) to
execute on behalf of the County, lntercounty Services Contract #24-889-3 with Alameda
County (for its Health Care Services Agency/Meatal Health Services) in the amount of
306,600, for the period from July 1, 1999 through June 30, 2000, for the provision of
psychiatric treatment services for seriously emotionally disturbed and behaviorally
disordered adolescents. This Contract includes a six--month automatic extension through
December 31, 2000 in the amount of$153,300.
FTSCAj�_IMPACT.
This Contract is funded 100% by Mental Health Realignment.
This Mental Health Program supports the following Board of Supervisors community outcomes:
"Children Ready for and Succeeding in School" , "Families that are Safe, Stable and
Nurturing" , and "Communities that are Safe and Provide a High Quality of life for Children
and Families" Expected program outcomes include an increase in positive social and
emotional development as measured by the Child and Adolescent Punctional Assessment Scale
(CAFAS) and placement at discharge to a lower level of care.
Wader this Interccunty Services Contract, Alameda County will provide up to four beds for
the Subacute Treatment for Adolescents with Rehabilitation. Services ;(STARS) Program for
County-referred adolescents who would otherwise be hospitalized at Napa State Hospital.
These STARS Program, clients receive residential care, intensive day treatment, non-public
school education, and acute psychiatric health facility services, as required.
On December 8, 1998, the Board of Supervisors approved Tntercounty Services Contract
#24-889-2 with Alameda County, for the period from ,lily 1, 1998 through ,rune 30, 10,99,
(which Included a six-month automatic extension through Dece:ber 31, 1999) for the
provision of psychiatric treatment services. Approval of sntercounty Services Agreement
24-889-3 will replace the six-month automatic extensionn of the print Contract arca allow

Approve and authorize the Health Services Director, or his designee
(Donna Wigand) to execute on behalf of the County, Contract
#24-795-6 with Christina C. Koch, for the period from October 1,
1999 through June 30, 2000, in the amount of $55,750, for the
provision of consultation and technical assistance to the Department
with regard to coordination of Mental Health Vocational Services
Programs.
FISCAL, IMPACT:
This Contract is 100% Mental Health Realignment funded.
On October 13, 1998, the Board of Supervisors approved Contract
#24-795-5 with Christina C. Koch, for the period, from October !,
1998 through September 30, 1999, for the provision of consultation
and technical assistance to the Department with regard to
coordination of Mental Health Vocational Services Programs.
Approval of Contract #24-795-6 will allow Contractor to continue
providing services through June 30, 2000.

(Wendel Brunner, M.D. ) to execute on behalf of the County, Novation
Contract #22.219-26 with Compass Group USA, Inc. , for the period from
July 1, 1999 through ;dune 30, 2000, with a payment limit of $1, 1148, 718,
for the provision, of congregate and home-delivered meals for the County'' s
Senior Nutrition Program, This Contract includes a four-month automatic
extension. through October 31, 2000, in the amount of $382 , 9016.
FISCAL IMPACT:
yhi Contract is Federally funded through the State under Title III C-1
and. Title III C-2 of the Federal Older Americans Act of 1965 . .No County
funds are required.
This Contractor was selected to provide meals for the Senior Nutrition
Program through a competitive bid processconducted by the County' s
General Services Department (Purcbasi ng Services)
On August 11, 1998, the Board of Supervisors approved Contract #22-219-25
with Compass Group LSA, Inc. , for the period from July 1, 1998 through
June 30, 1999, to provide meal services for the Senior Nutrition Program.
Approval.. of Nrovation Contract 22--21-9-26 replaces the three-month
automatic contract extension under the prior Contract and will allow
Compass Group SSA, Inc . to prepare and deliver an an daily
average of 1, $93 meals in bulk regular diet, prepackaged cold dinner,,
prepackaged regular diet, and prepackacred sodium-restricted diet -meals on
250 County-de ionated serving days . The Contractor delivers the :deals to
locations throughout the County. The unit prices for the meals to be
provided by the Contractor during FY 1999-2000 are as follows:
Burk Regular Diet Meals ;2 . 69 per meal
Prepackaged Regular and Cold Dinner deals $2 . 97 per peal
Prepackaged Modified Diet Meals $3 . 06 per meal .
