SPECIFIC REQUEST($)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION —
Approve and authorize the Health Services Director, or his designee;
(Frank Puglisi, Jr. ) to execute on behalf of the County, Contract
##26--969 with. Gupta Etwaru, M.D. , (Specialty. Ophthalmology) for the
period. from August 1-6, 1999 through August 31, 2000, to he paid as
follows :
$15,833 .33 per month, for the provision of Ophthalmology services
including consultation, clink coverage, training, cin-call services
and ,medical and./or surgical procedures, NOT TO EXCEED $190,,000 .
FISCAL, IMPACT
Cost to the County depends upon utilization. As appropriate,
patients and/or third party payers will be bill®d for services .
BACKGROUND/REASON(S) FOR RECOMMENDATIONS:
For a number of years the County has contracted with Medical and
Dental Specialists to provide specialized professional services
which are not otherwise available in its hospital and clinics .
Under Contract ##26-969, Dr. Gupta Etwaru. will 3Drovide Ophthalmology
services for Contra Costa ?regional Medical Center and Centra Costa
Health Centers through August 31, 2000 .

Approve and authorize the Health Services Director or his designee (Donna
Wigand) to execute on behalf of the County, Contract 24-778-7 with
FamiliesFirst, Inc. , for the period from July 1, 1999 through June 30, 2000,
in the amount of $552 , 151., for the provision of intensive bay Treatment and
Mental Health Services for Seriously Emotionally Disturbed (SED) Children
at tact. Diablo High School.
FISCAL IMPACT:

CHILDRE 't S ,IMPACT STATEMENT:
This dental Health Services Contract will support the following Board of
Supervisors' community outcomes: "Children Ready For and Succeeding in
School" "Families that are. Safe, Stable, and Nurturing" ; and "Comimuni.ties
that are Safe and Provide a High Quality of Life for Children and Families" .
Expected program outcomes include an increase in positive social and
emotional development as measured by the Child and. Adolescent Functional
Assessment Scale (CAFAS) .
BACKGROUND/REA SNI S) FOR RECOMMENDATION(S)_:
This Contract meets the social needs of County's population in that it
providess intensive day treatment and medication support for SED children to
reduce the need for out-of-hone placements.
On February 2 , 1999, the Board of Supervisors approved Novation Contract
#24-778-6 with FamiliesFirst, Inc. , for the period from July 1, 1998 through
June 30, 1999 for the provision of intensive day treatment and medication
support services for seriously emotionally disturbed children at Oakgrove
Crisis Residential/'Day Treatment Programs.
Approval of Contract #24-778-7 will allow the Contractor to continue
providing services through June 30, 2000.

RECOMMENDATION(S).-
Approve and authorize the Health Services �ti_rector, or his designee '
(Frank Buglis ) , to execute on .behalf of the County, Amendment 426-162-
14 (University 93-03684V) with the Regents of the University of
Cay i-f�orn a (U. C, Davis School of Internal Medicine Department) , to
ar:end Contract #26-162-9, effective June 30, 999, to Increase the
Contract Payment Limit by $20, 000 from $640, 00C. to a new total of
$660, 000, to provide additional oncology/hematology services .
FISCAL IMPACT:
This Contract is funded in the Department ' s Enterprise I Budget. The
services provided under this agreement are billable to patients and
appropriate third party payors.
BACKGROUND/REASON(S) FOR RECO ENDATION(S) :
On July 13 , 1993 , the Board. of Supervisors approved Contract. #26-16.2-9
(as amended by Amendment Agreements #26-162-10 through #26-11 62-13) with
the Regents of the University of California (U.C. Davis School of
nternay MedicineDepartment) , for the period from July 1, !993 through'
June 30, 2000,< for professional oncology services for patients treated'
at Contra Costa Regional Medical Center and. Contra Costa wealth'
Centers .
For several years, the .Board of Supervisors has approved contracts with
the Regents of the University of California (U. C. Davis School of
Medicine) for professional services in several medical specialties .
Services are 'provided' to 'county' s inpatients and outpatients at Contra
Costa Regional Medical Center and Contra Costa Health Centers by;,
physicians who are U.C. Davis School of Medicine faculty members .
Approval of Amendment #26-162-14 will allow Con-tractor to provide'
additional Hematology/Oncology clinic services under this Agreement'
through June 30, 2000 .

Frank Puglisi) to execute on behalf of the County, Unpaid Student
Training Agreement #26-1.26.2 with Contra Costa Community College
District, for the period from September 1, 1999 through August 31,,
2004, to provide field instruction and experience for students .gin
the College District's Nursing, Emergency Medical 'technician, and
Medical Assistant/Office Technician Programs.

BACKGR0UNDLREA; ONE S) FCR RECOMMENDATION(S) :
€3n November 8, 1994, the Board of Supervisors approved Unpaid
Student Training Agreement #26-126-1 with Contra Costa community
College District for the period from September !, 1994 through
August 31, 1999.
Supervised field work experience is considered to be an integral
part of both educational and professional. preparation. As a
teaching hospital, Contra Costa Regional Medical Center can provide
the requisite te• field education, while at the same time taking
advantage of the students' services to County's patients.
Approval of Unpaid Student Training Agreement #26-126-2 will allow
students from Contra Costa Community College District to continue
receiving clinical and field experience in Nursing, Emergency
Medical Technician, and Medical Assistant/Office Tecl nic an programs
through August 31, 2004 .

program services to homeless single ren and families at the San
Joaquin 11 apartments in Richmond.
Approval of Contract Amendment Agreement 25-006--6 gill allow the
Contra t0r to continue providing services through October 31, 1999.
