Citizens, Inc. , in the amount of $236,934, for the period. from July 1., 1999 through Jane 30, 2000, for provision of a day treatment program to high risk, delayed, or emotionally disturbed children at the Lynn Center. This Contract includes a six-month automatic extension through December 31, 2000, in the amount of 11. ,497a ACT This Contract is funded as follows: 1.07,404 Federal Medi-Cal (FF ) Funds _219,190 dental Health Realignment Funds 236,994 TOTAL CONTRACT TWYMENT LIMIT This program supports the following Board. of Supervisors, community outcomes: l) Children Ready For and Succeeding in School; 2) Families that are Safe, Stable, andNurturing; and. 3) Communities that are Safe and Provide a High Quality of Lire for Children. and Families. Expected program outcomes are: a) increase and maintain school attendance as measured by school records; b) increase in positive social and emotional development as measured by the Child and Adolescent Functional Assessment Scale (CA AS) ; c) Increase in family satisfaction as measured by the Parent Satisfaction Survey; d) Decrease in cart-of-home placements as measured by mental health and social services statistics; and a) Placement at discharge to a. lower bevel of case. For several years, this Contractor has been operating a day treatment program, for preschoolers with measurable delays in interpersonal, social/emotional, Language and cognitive development for children who are a4 rise for such delays, included are abased, developmentally delayed, emotionally disturbed and environmentally deprived children who do not meet the criteria for any categorical. funding .source for ;services On January 5, 1999, the Board of Supervisors approved Novation; Contract #24-707-35 (as amended by Administrative Amendment Agreement #24-707"36) for the period from July 1, 1998 through June 30, 1999, which included a six-month automatic extension, throe& December 31, 1999. This Novation Contract replaces the six-month automatic ext-ension of the prior Contract, and allows the Contractor to continue providing services through Jane 3 :, 2000.
