Approve and authorize submission of Funding Application ##29-45.5-9 to the state De,oartment of Mental Health in the amount of $39, 982, for FY 1999-2000, to continue PATH Formula Grant Funding under the Stewart B. McKinney Homeless Assistance Amendments Acts of 1990 (PD 101-645) . B. Authorize the Health Services Director, or his designee (Donna Wigand) to sign required assurances and certifications regarding debarment and suspension, a drug-free workplace, lobbying, and compliance with Federal reTiirements, and to make any minor technical adjustments to the Grant Application that may be required by the State Department of Mental Health. F-SCAL IMPACT. Approval by the State, Department of ?ental Health of this application will result in $39, 982 of Federal PAH Grant Funds for the continuation of outreach services to the homeless mentally disabled. The application requires an additional non-Federal match ($13, 946) which will be provided through existing County Real-4-ment funds. Approval of this application 429-455-9 will continue the PATH project operated. by Phoenix Program, Inc. , under County's Contract #24-365, for the provision of meat-al health outreach services for the icmeless Mentally Ill (two Multi-Serv-_ces Centers and one 20-hed Shelter/Multi- Services Center) as San Pablo, Concord and Antioch. The Federal PAT: Grant Application for 1-999--2000 was reviewed and recommended for approval by the County' s Mental Health Commissicn or June 2, 1959, In order to meet the deadline for submission, the app-kation has beer, forwarded to the State, but subject to Board approval. Three certified copies of the Board Order authorizing submission of the application should be returned to the Contracts and Grants Jnit for submission to the State. _ .
