NOTE that the new acute care hospital has provided the means for staff to provide the highest quality and most economical acute care to residents and employees of Contra Costa County. 2. ACKNOWLEDGE that construction will soon begin on a new Clinical and Public Health Service Laboratory at the medical center as well as parking improvements to main patient lot, upper lot, and emergency room lot. 3. RECOGNIZE that older buildings currently occupied by outpatient, support, and administrative services are both substandard and operationally inefficient. 4. ACKNOWLEDGE that in order to deliver quality comprehensive and cost effective health care to its patients the Health Services Department needs to plan for long term development of the entire medical center campus. 5. RECOGNIZE that in August 1998 the Board of Supervisors AUTHORIZED the development of a master plan for the medical center campus that included the programming of a new Ambulatory Care Facility. 6. ACKNOWLEDGE that at least six alternatives were developed for the master plan, and the most efficient, cost effective alternative was selected by staff for further development and recommendation to the Board. CONCUR with staff recommendations that Scenario B (Package 111) is the most feasible alternative. Scenario B includes new Specialty Clinics (a.k.a. Ambulatory Care Facility), a hospital conference room, demolition of B, C, and D wards, removal of vacant modular buildings on the south side of the old hospital (CT Scan), and remodeling of South Wing and E Ward for additional functions. 8. AUTHORIZE the County Administrator to proceed with the development of Scenario B that includes a new 55,818 gross square foot Ambulatory Care Facility. 9. ACKNOWLEDGE that after a formal competitive selection process staff recommended to the Board, and on April 20, 1999 the Board AUTHORIZED, negotiations with the highest qualified architectural firm to design a new Ambulatory Care Facility. 10.APPROVE and AUTHORIZE the County Administrator or his designee to execute a Consulting Services Agreement with Anshen + Allen Architects Inc (the highest qualified architectural firm) for design of a new Ambulatory Care Facility in the amount of$1,268,650. 11.AUTHORIZE the County Administrator or his designee to issue written authorizations for extra work, provided that the cumulative total extra cost for all such authorizations does not exceed $25,000. FINANCIAL IMPACT: The current estimated total project cost for development of Scenario B is $24.6 million. Based on current budget forecasts, it is anticipated that $5.6 million of the costs of the new health center will be funded with bond funds remaining after the completion of the new CCRMC hospital and clinical lab. The remaining $19 million will need to be funded through the issuance of additional lease revenue bonds some time next spring. Earlier this year, the County refinanced and restructured existing Health Services Department debt which reduced annual debt service to the Department by $1.4 million per year; assuming the County issues $19 million in 30-year lease revenue bonds at an interest rate of 6%, annual debt service for this new project will be approximately $13M, or a net increase of $300,000 per year as compared to the old debt service before the refinancing and restructuring took place. BACKGROUND: The acute care hospital at the Contra Costa Regional Medical Center was opened for business in January 1998. This new hospital provides a state-of-the-art facility for Contra Costa County residents and employees. It replaced an obsolete, seismically unsound facility that had outlived its useful life. Construction of the new hospital was completed under budget and as a result the County was able to fund other miscellaneous projects, and make funds available for the proposed Ambulatory Care Facility. In August of 1998 the County began the development of a comprehensive master plan for the CCRMC campus and a preliminary design program for replacement of the specialty care clinics. As part of the original project, plans are already complete and construction will soon begin on a new Clinical and Public Health Service Laboratory and associated parking. After implementation of these projects, the County will be fully equipped with modern health care facilities on the Martinez campus to serve the needs of central Contra Costa. On April 20, 1999 the Board of Supervisors approved recommendations of the Architect Selection Committee for the new Ambulatory Care Facility. The Board also authorized negotiations with the two highest qualified firms in order of preference. The top two firms in preferred order were Anshen + Allen Architects, Inc and Kaplan McLaughlin Diaz (architect of the acute care hospital). The top ranked firm, Anshen + Allen Architects Inc., has negotiated a price proposal within the county's budget allocation for this work. This firm has successfully completed such projects as the new Main Hospital for the Santa Clara Valley Medical Center, an expansion for Children's Hospital in Oakland, and the Tang Center at the University of California at Berkeley. The executed contract will include the Schematic Design Phase for $190,000, Design Development Phase for $255,000, the Construction Documentation and Bidding Phase for $570,000, and Construction Administration Phase for $255,000.
