RECOMMENDED ACTION:
Approve and authorize the Health Services Director or is designee
(,ewis Bascalli) , to execute on behalf of the County, Contract
Amendmsent Agreement 4122-276-12 with the City of Richmond (Fire
Department) , effective June 30, 111999, to amend the Contract #22-276-
11, to increase the aut-cmatic extension -Day;gent jimli t by -47, 500
from $95, 0010 to a new automatic extension payment limit of $142 , 500
and to extend the ter-i through December 31, 1999 .
FINANCIAL IMPACT:
This Contract is included in the Health Services Department' s Budget
and is funded by AB 2185 inspection fees .
REASONS FOR R Cts E DATIONS ZBACKGRO` ND:
On March. 17, 1998, the Board of Supervisors approved Contract
422-276-11 with the City of Richmond (Fire Department) , for the
period from July 1, 1997 through June 30, 1998 (which included a
one-year automatic extension through June 30, 1999) , to provide AB
2185 inspections .
This purpose of this amendment is to extend the Contract so that
services are not disrupted, pending final negotiations between the
Department' s Hazardous Materials Divisions and the City of Richrrond.
Approval of Contract Amendment Agreement #22-275-12 will allow the
Contractor to continue providing services under the autocratic
extension, through December 31, 1999 .

Funding for this Contract is .included in the Department's Fiscal.
Year 1999-2000 Budget, and the source of funding is 100% Mental
Health Realignment, partially off-set by Medi-Cal, Medicare, and
insurance billed by the Department

#24-403-5, with Jon Whalen, M.D. , for the period from July 1, 1998
through June 30, 1999, for the provision of psychiatric care and
treatment for adolescents placed by County at Walnut Creek Hospital,
or Esther authorized hospital facilities.
.Approval of Contract #24-403-6 will allow the Contractor to continue
providing services through June 30, 2000.

On October 6, 1998, the Board sof Supervisors approved Novation
Contract 24-21.3-31 (as amended by Administrative Amendment,
Agreement #24-213-32) for the period from. July 1, 1998 through June
30, 1999, to provide information and referral, consultation and
education, and outpatient mental health services for West Contra
Costa County (Familial unidas Counseling Center)
This Novation Contract replaces the six-month automatic extension
of the prior Contract and allows the Contractor to continue
providing services through June 30, 2000.

Approve and authorize the Health Services Director or his designee (Frank
Puglisi, Jr. ) , to execute on Behalf of the County, Contract Amendment Agreement
426-904-9, with Steph6n D. Weiss, M.D. (Specialty: General Surgery) , effective
May "1., 1999, to amend Contract 426-904-8, to ;codify time Contract payment
provisions to allow the Contractor to provide additional. care-call General
Surgery coverage services .
FECAL IMPAC'l`
This amendment will modify the on-call coverage set forth in the Additional Provisions of
Contrast #2E-904-8, paragraph 1. (Payment) , subparagraph b. to read as follow:
"1.b. i) S 9ao.90 per :month for on-call coverage performed through April 30, 1999,
2) 13,6000AC per month for on-call coverage performed during the month of May 1999,
3) 11,6jO.Q per month for on-call coverage performed during the months of June 1999
through December 19999 and
40 .95P.10 per month for on-call coverage performed during the month of January 2000."
Cast to the County depends upon utilization. As appropriate patients and/or third party payors
will be billed for services.
REASONS FOR REcommENDATIONSAACKGROUND:
On January 5, 1999, the Board of Supervisors approved Contract 26-904-8 with
Stephen 1). Weiss, M,D. , for the period from January 15, !999 through January
31, 2000, for the provision of General Surgery services .
At the request of the County, far. Weiss has agreed to provide additional on-
call General Surgery coverage, in addition to his regular on-call coverage
services for Contra Costa. Regional. Medical Center and Contra Costa. Health
Centers, while the Depart-rent continue .its recruitment process to hire a full
time General Surgeon.
Approval of Contract Amendment Agreement 26-9014-9, will allow Dr. Weiss to be
properly compensated for additional on-call. General Surgery services provided
through January 31, 2000 .
