.Approve and authorize the Health Services Director, or his designee
(Donna Wigand) to execute on behalf of the County, Novation Contract
#24-648-9 with Telecare Corporation, in the amount of $185, 300, for
the period from July 1, 1999 through June 30, 2000, for provision
of mental health treatment services to Contra Costa County geriatric
clients at Morton Bakar Center. This Contract includes a six-month
automatic extension through December 31., 2000, in the amount of
$92, 650.
This Contract is 100% funded by dental Health Realignment funds
On December 1, 1998, the Beard of Supervisors approved Contract
#24-648-8 with Telecare Corporation for the period from July 1., 1998
through June 30, 1999 (with a six-month automatic extension through
December 31., 1999) for the provision of mental health treatment
services to Contra Costa County geriatric clients.
This Novation Contract replaces the six-month automatic extension
of the prier Contract and allows the Contractor to continue
providing services through June 30, 2000 .

(Chuck Deutschman) to execute on behalf of the County, Contract
Amendment Agreement #24-42922 with Ujima Family Recovery Services,
effective dune 1, 1999, to amend Novation Contract #24®429--20 (as
amended by Administrative Amendment Agreement #24°-429-21.) to
increase the Contract Payment Limit by $7, 876, from $31.9,652 to a
new tote, of $327,528 and to increase the Automatic Extension
Payment Limit by $3,938, from $159,826 to a neer total of $163,764 .

On. January 19, 1999, the Board of Supervisors approved. Novation
Contrast #24-429-20 (pis amended by Administrative Amendment
Agreement #24--429-21) with Ujima Family Recovery Services for the
period from July 1, 199£3 through June 30, 1999 (with a six-month
automatic extension through December 31, 1999) for the provision of
substance abuse residential program services (Perinatal Set-Aside
Federal Block Grant Project) .
Approval of Contract Amendment Agreement #24-429-22 will allow the
Contractor to provide services to additional clients participating
in County e s Alternative to General Assistance Program (AGAP) through
June 30, 1999
