This Contract includes a six-month automatic extension through December 31., 2000, in the amount of $92, 650. This Contract is 100% funded by dental Health Realignment funds On December 1, 1998, the Beard of Supervisors approved Contract #24-648-8 with Telecare Corporation for the period from July 1., 1998 through June 30, 1999 (with a six-month automatic extension through December 31., 1999) for the provision of mental health treatment services to Contra Costa County geriatric clients. This Novation Contract replaces the six-month automatic extension of the prier Contract and allows the Contractor to continue providing services through June 30, 2000 .
