RECOMMENDATION(S) :
Approve and authorize th~e Health Services Director, or his designee,
(Frank Buglisi, Jr. ) to execute on behalf of the County, Contract
26-945-1 with Rex Moulton-Barrett, M.D. {specialty: Hand and
Plastic Surgery} fo-r the period from October 1, 1998 through
September 30, 1999, to be paid as follows :
a. $13, 666 .67 monthly salary, for consultation, training, and
medical and/or surgical procedures, not to exceed a total
payment of $164 , 000; and
b. 950 .00 per month for on-call coverage, consisting of no
more than every other night and no more than sixteen %11611
nights per month. In the event that on-call coverage exceeds
sixteen (16) nights in a month, County shall pay Contractor
65 . 00 for each night in excess of sixteen (16) ; and
C . $ 380.00 per occurrence when Contractor is required to come
to the Hospital during on-call hours for perform a surgical
procedure; and
d. An amount NOT TO EXCEED $151. 00 to reimburse Contractor for
educational activity expenses incurred, during the term of this
agreement .
FISCAL IMPACT:
Cost to the County depends upon utilization. As appropriate,
patients and/or third party payors will be billed for services .
BACKGRROUNDIR.EASON(S) FOR RECOMMENDATION(S) :
On October 7, 1997, the Board of Supervisors approved Contract ##26-
945 with Rex Moulton-Barrett, M.D. , for the period from October 1,
1997 through September 30, 1998, for the provisions of Hand and
Blastic Surgery services .
Approval of Contract ##26-945-1, will allow Dr. Moulton-Barrett to
continue providing professional hand and plastic surgery services
for Contra Costa Regional Medical Center through September 30, 1999 .

provisions of temporary nursing registry ,services .

Department ' s Enterprise I budget .

In October 1997, the County Administrator approved and the Purchasing
Services Manager executed Contract #26--318 with American. Mobile Nurses,
for the period from August 1, 1997 through July 31, 1998 , For several
years the County has contracted with nursing registries to provide
temporary licensed nursing personnel to assist Conga Costa Regional
Medical Center and Health Centers during peak loads, temporary absences
and emergency situations . There is a nationwide nursing and healthcare
staff shortage, and in spite of persistent efforts to recruit nurses
and other healthcare staff, the Department has experienced great
difficulty in filling any vacant. positions . Therefore, the Department
has had to rely heavily on the registry services in order to provide
quality nursing care for patients .
Approval of Contract #26-318-1, will allow the Contractor to continue
providing services through July 31, 1999 .

Training Agreement #26-341. with Regents of she University of California,
on behalf of University of California, San Francisco, School, of Medicine,
for the period from July 1, 1.998 through June 30, 2901 for the provision,
of clinical experience for physical therapy students.

BACKGRQU/MASON,�S FOR RECOMMENDATIPNCSL :
V'he purpose of this agreement is to provide medical students with the
opportunity to integrate academic knowledge with application skills and
attitudes at progressively higher levels of performance requirements and
responsibility. Supervised field work experience in medicine is
considered to be an integral part of bots the educational and
professional. preparation. As a teaching hospital, Contra. Costa Regional
Medical Center and the Contra Costa Health Centers dart provide the
requisite field education, while at the same time, taking advantage of
the students' .services to patients .
Approval of Unpaid Student Training Agreement #26-341 will allow
University of California, San Francisco students to receive field
instruction and experience at Contra Costa Regional Medical Center and
the Contra Costa Health Centers through June 30, 2001.

Wigand) to execute on behalf of the County, Contract #24-868-2 with
Telecare Corporation, for the period from July 1, 1998 'through June 30,
1999, for inpatient psychiatric services, to be paid in accordance with
the following rate schedule:
$472 . 99 for each Medi-'Cal "Acute Psychiatric Day of Service" ,
830 . 21 for each Non-Medi-Ca' "Acute Psychiatric Day 'of Service" , and
$268 . 63 for each "Administrative .Day"
FISCAYa IXPACT:
This Contract is included in the Health Services budget and is funded by
acute psychiatric inpatient consolidation funds .
BACXgAP� REASON FOR RRCOMM ATION S :
h1.s Contract is part of the Department' s cast saving plan to reduce the
number of high-cost Mate Hospital bergs by developing alternative
placements .
On June 24, 1997, the Board of Supervisors approved Contract #24-868-1.
with Telecare Corporation for provision of inpatient psychiatric
treatment services to County' s patients at its Telecare ola.no/
Psychiatric Health Facility as an alternative to utilization at Napa
State Hospital, for the period frog July 1, 1997 through June 30, 1998 .
Approval of Contrast #24-868-2 will allow the Contractor to continue
providing services through June 30, 1999 .

Amendment Agreement #26-236-8, to amend Contract #26-236-6 (as amended by
Contract Amendment Agreement #26-236-7) with Judy Windrix, A.R.T. (dba
Judy Windrix Transcription) effective September 30, 1993, to extend the
term of the Contract through December 31, 1998 and increase the Contract
payment limit by $50, 000, from $150, 000 to a new total payment li mit of

This contract is included in the Health Services Department Enterprise I
budget.
BACMROUND L REAS ON(S) FOR RECOMMENDI TION(S)
On December 2, 1997, the Board of Supervisors approved Contract #26-236-6
with Judy Windrix, A.R.T. (dba Judy Windrix Transcription) for the period.
from December 1, 1997 through May 31, 1998, for provision of medical
transcription services for Centra Costa Regional Medical Center.
Subsequent Contract Amendment Agreement #26-236-7 extended the term of
the Contract through September 3€7, 1998.
Expert medical transcription services are necessary to meet the standards
for Medical Record Services set forth in the J.C.A.H.t1. Accreditation
Manual and to address the problem of illegibility on the part of medical
professionals for which Centra Costa Health Services has been criticized
in State Department of Health Services PHP medical audits.
Approval of Contract Amendment Agreement #26-236-8 will allow the
Contractor to continue providing services through December 31, 1998 .
