Herbert Chew
SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION
I. RECOMMENDED ACTION:
Approve and authorize the Health Services Director, or his designee
(Donna Wigand) , to execute on behalf of the County, Novation
Contract #24-834-1 with Herbert Chew, in the amount of $43, 600, for
the period from July 1, 1996 through June 30, 1997 , to provide
mental health case management services' for the Intensive
Intermittent Intervention Program.
FINANCIAL IMPACT:
This Contract is funded as follows:
$21,800 Federal FFP Medi-Cal Funds
21,800 State EPSDT Funds
$43 , 600 Total
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
Herbert Chew has been providing mental health case management
services for the Intensive Intermittent Intervention Program under
the automatic extension of Contract #24-834 .
Novation Contract #24-834-1 replaces the automatic extension under
the prior Contract, and will allow the Contractor to continue
providing services through June 30, 1997 .
