RECOMMENDATION 1 . APPROVE: the Nineteenth Amendment of the Consulting Services Agreement with Kaplan/McLaughlin/Diaz, and AUTHORIZE the County Administrator or his designee to execute the Amendment 2 . AUTHORIZE: the County Administrator or his designee to issue written authorizations for extra work, provided that the cumulative total extra cost for all such authorizations shall not exceed $25, 000 . 00 . II . FINANCIAL IMPACT This Amendment increases the Architect' s payment limit by up to $23 , 700 for services required to respond to changes . Funds in the capital account established for the project are sufficient to cover the amount encumbered by this Amendment from bond proceeds specified to the project . III . BACKGROUND This change represents five extra services authorizations . EFFECTIVE DATE AND PARTIES Effective on September 10, 1996, Kaplan/McLaughlin/Diaz, a California corporation . (hereinafter called "Consultant") , and the County of Contra Costa, a political subdivision of the State of California (hereinafter called "Public Agency") , mutually agree as follows: 2. PURPOSE Effective March 3, 1992, the parties entered into a written contract entitled "Consulting Services Agreement" (referred to as the "Agreement") , which covered architectural services for a replacement hospital at 2500 Alhambra Avenue, Martinez. The Agreement has previously been amended as follows: Amendment No. 1 dated December 8, 1992; Amendment No. 2 dated December 14, 1992; Amendment No. 3 dated December 16, 1992; Amendment No. 4 dated April 6, 1993; Amendment No. 5 dated April '29, 1993; Amendment No. 6 dated June 9, 1993; Amendment No. 7 dated April 19, 1994; Amendment No. 8 dated June 2, 1994; Amendment No. 9 dated November 29, 1994; Amendment No. 10 dated December 30, 1994; Amendment No.11 dated June 8, 1995; Amendment No. 12 dated June 20, 1995; Amendment No. 13 dated July 1, 1995; Amendment No. 14 dated January 9, 1996; Amendment No. 15 dated March 12, 1996; Amendment No. 16 dated May 7, 1996; Amendment No. 17 dated July 9, 1996 and Amendment No. 18 dated July 23_1996. - The County desires to further amend the Agreement to expand the scope of services and to revise the payment limit accordingly. 3. AMENDMENTS TO AGREEMENT A. In the Agreement, Section 1(e) , change the payment limit from $5,296,411.02 to $5,320,111.02, an increase of $23,700.00. B. In Appendix B, Paragraph D.3, add the following: be Provide architectural and engineering services required to design a small bottle storage room as described in Extra Work Proposal No. 66 dated July 16, 1996 (copy attached) . Authorized Limit: $3,200.00. bf Provide architectural and engineering services required to redesign the film processor and developer layout (OCR #21.1) as described in Extra Work Proposal No. 67 dated July 16, 1996 (copy attached) . Authorized limit: $16,800.00. Al bg Provide architectural and engineering services required to coordinate structural attachments for the County provided bulk oxygen storage tank as described in Extra Work Proposal No. 68 dated July 16, 1996 (copy attached) . Authorized limit: $1,800. 00. bh Provide architectural and engineering services to delete oxygen from medical utilities unit U-22 in Central. Supply (OCR #12) as described in KMD's Extra Work Proposal No. 49 dated July 16, 1996 (copy attached) . Authorized limit: $1,100.00. bi Provide miscellaneous adjustments to patient rooms (OCR #24) as described in KMD's Extra Work Proposal No. 69 dated July 16, 1996 (copy attached) . Authorized limit: $800.00. Total Amendment No. 19 $23,700.00 4. REMAINING PROVISIONS Except for the changes made by this Amendment and prior amendments, all provisions of the Agreement shall remain in full force and effect. A . 5.
