SUBJECT: Approval of Contract Amendment Agreement #22-525-4 with Delta 2000
SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION
I. RECOMMENDED ACTION:
Approve and authorize the Health Services Director, or his
designee (Wendel Brunner, M.D. ) , to execute on behalf of the
County, Contract Amendment Agreement #22-525-4, effective June 1,
1996, to amend Contract #22-525-3 with Delta 2000, to increase the
Contract Payment Limit by $10, 000, from $29, 755 to a new total
payment limit of $39,755.
II. FINANCIAL IMPACT:
This Contract is funded 100% by Federal funds. No County funds
are required.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
On December 5 1995, the Board of Supervisors approved Contract
#22-525-3 with Delta 2000, for the period from October 1, 1995
through September 30, 1996, for food stamp nutrition education and
outreach services for the Contra Costa County Cooperative
Nutrition Education Project.
Approval of Contract Amendment Agreement #22-525-4 will allow the
Contractor to provide additional services through September 30,
1996.
