Costa County DATE: July 23 , 1996 SUBJECT: APPROVE THE 18th AMENDMENT FOR KAPLAN/McLAUGLIN/DIAZ MERRITHEW MEMORIAL HOSPITAL REPLACEMENT PROJECT (WH580B) SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION I . RECOMMENDATION 1 . APPROVE: the Eighteenth Amendment of the Consulting Services Agreement with Kaplan/McLaughlin/Diaz, and AUTHORIZE the County Administrator or his designee to execute the Amendment 2 . AUTHORIZE: the County Administrator or his designee to issue written authorizations for extra work, provided that the cumulative total extra cost for all such authorizations shall not exceed $25, 000 . 00 . II . FINANCIAL IMPACT -` This Amendment increases the Architect' s payment limit by up to $45, 400 for services required to respond to changes . Funds in the capital account established for the project are sufficient to cover the amount encumbered by this Amendment from bond proceeds specified to the project . III . BACKGROUND This change represents nine extra services authorizations . One item is the result of changes to duct acoustics in response to CONTINUED ON ATTACHMENT: YES SIGNATURE: LAI? BACKGROUND (continued) equipment noise level changes . Another item is for electrical and infrastructure changes required to accommodate the recently designed data/telecommunications system. Seven items are for a variety of design changes needed to respond to operational changes or to accommodate changing hospital equipment .
