SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION
I. RECOMMENDED ACTION:
Approve and authorize the Health Services Director, or his
designee (Art Lathrop) , to execute on behalf of the County,
Contract Amendment Agreement #23-134-8 with Aquarius Public
Management, Inc. , effective July 1, 1995, to amend Short Form
Service Contract #23-134-6 (as extended by Contract Extension
Agreement #23-134-7) , to increase the Contract Payment Limit by
$25, 000, from $24,725 to a new total of $49, 725.
II. FINANCIAL IMPACT:
This Contract is funded by CSA EM-1 (Measure H) funds.
III. REASONS FOR RECOMMENDATIONS BACKGROUND:
This Contractor has been providing consultation and technical
assistance to the Department's Emergency Medical Services (EMS)
regarding implementation of a computerized system to interlink
emergency ambulance, fire/paramedic and "911" dispatch centers
under Short Form Service Contract #23-134-6 (effective July 1,
1994 through June 30, 1995) , as extended by Contract Extension
Agreement #23-134-7.
Approval of Contract Amendment Agreement #23-134=8 will allow the
Contractor to provide additional services for the development and
implementation of an Emergency Medical Network to link hospitals,
County and other key EMS resources, through June 30, 1996.

AMENDMENT SPECIFICATIONS
Number-23-134-8
In consideration of Contractor's agreement to provide additional services
under Contract #23-134-6 (as extended by Contract Extension Agreement #23-
134-7) , County agrees to increase the Contract Payment Limit. County and
Contractor agree, therefore, to modify said Contract as specified below,
while all other parts of the Contract remain unchanged and in full force and
effect.
1. Payment Limit Increase. The Contract Payment amount set forth in
Paragraph 5. (Payment Limit) , is hereby increased by $25, 000, from $24,725 to
a new total payment limit of $49,725.
2 . Modifications of County Obligations. Paragraph 6. (County
Obligations) , subparagraph a. , is hereby deleted and replaced with a new
subparagraph a. , to read as follows:
" [X] a. FEE RATE: $ 125. 00 per service unit: (X) hour
NOT TO EXCEED a total of 415 service unit(s) . "
3 . Modification of Work Plan. The Work Plan described in Additional
Provisions Paragraph 3. (Service Activities) shall be modified to specify the
additional services which the Contractor has agreed to provide pursuant to
this Amendment.
