SUBJECT: SUBMITTAL OF GRANT PROPOSAIS TO EAST BAY COMMUNITY FOUNDATION
SPECIFIC REQUEST(S)OR RECOMMENDATION(S)&BACKGROUND AND JUSTIFICATION
RECOMMENDATIONS
Approve the submittal of two grant proposals to the East Bay Community Foundation to conduct
Healthy Neighborhood programs in East and West County to help county-funded health programs
in those locations make the transition to managed care.
FISCAL IMPACT
Total request is $225,000. The East County grant proposal, if awarded, would result in
$75,000 of new revenue to the County. The West -County proposal, -if awarded, would result
in $150,000 of new revenue to the County. gAn, add.itional $52,368.00 of in-kind support from
existing programs will complement the grant.
BACKGROUND
As managed care becomes a guiding force. for health care programs, MediCal patients will no
longer be restricted to uLsing. county facilities,. but will be able to select from a range of
providers. In order for County facilities: to make the transition to this more competitive
approach, strategies must be devised to attract patients who have had positive experiences
with the County's health programs.
The proposed projects will focus on the catchment. areas of the Pittsburg Health Center and
the Center for Health in North Richmond and will.. build upon successful pilot efforts conducted
last year. The projects will work closely with-residents.of the two areas to identify
conditions in their neighborhoods which affect their health and help them to make the changes
they want. The project will create close working relationships between residents and the
staffs in the Health Centers. In the case of the Center for Health, the proposal.z- also
provides resources to further develop the program activities to be implemented at the Center.
Instead of highlighting deficiencies in these communities, which many researchers have done
already, these projects will focus on the capacities of these communities and will assist
residents to mobilize resources to improve their lives and the lives of their children.

support the individual and communifty to take increased responsibility for their health and
develop systems and infrastructure to support managed care.
