Recommendation:
Relieve the Health Services Department - Pittsburg and Richmond Health Clinics' cash shortages as follows pursuant to
Government Code Section 29390, as recommended by the Auditor-Controller:

Reasons for Recommendation/Background:
Pittsburg Health Center:
The petty cash fund has been short$5.00 since October 27, 1994. An additional$1.00 shortage was discovered on
March 31, 1995. On May 1, 1995, a reconciliation of receipts versus cash and checks identified a cash shortage of
$20.00 and that twenty receipts for patient payments totaling$302.92 were also missing. A complete report was filed
with the Pittsburg Police Department.
Richmond Health Center:
In two separate incidents, one in 1992 and one on April 27, 1995, monies and receipts were missing for four patient
payments totalling $100.00. It appears that the money and copies of the receipts were removed from the cash box
in the registration area

Pittsburg Public Health:
During the weekend of June 4-5, 1995, three petty cash boxes were broken into and their contents totalling $70.00
were stolen. A complete report was filed with the Pittsburg Police Department.
So far as can be determined, the shortages were not caused by negligence or fraud. All possible efforts to ascertain
the amounts of the shortages have been exhausted. New procedures have been implemented to avoid the occurrence
of similar incidents.
The Auditor-Controller has reviewed the documentation as to how the shortage occurred and has recommended that
relief of the shortage be granted.
