HomeMy WebLinkAboutMINUTES - 09121995 - C93 TO: BOARD OF SUPERVISORS FROM: Mark Finucane, Health Services Director Contra NQ Costa DATE: August 29, 1995 County SUBJECT: Approval of Funding Application #29-455-5 to the State Department of Mental Health for McKinney Homeless PATH Grant Funding in FY 1995-96 SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION I. RECOMMENDED ACTION: A. Approve and authorize submission of Funding Application #29-455-5 to the State Department of Mental Health in the amount of $91,472, for FY 1995-96, to continue two Mental Health Projects for Assistance in Transition from Homelessness (PATH Formula Grant Funding) under the Stewart B. McKinney Homeless Assistance Amendments Acts of 1990 (PL 101-645) . B. Authorize the Health Services Director, or his designee (Lorna Bastian) to sign assurances and certifications regarding debarment and suspension, a drug-free workplace, lobbying, and compliance with Federal requirements, and to make any minor modifications to the Grant Application that may be required by the State Department of Mental Health. II. FINANCIAL IMPACT: Approval by the State Department of Mental Health of this application will result in $91,472 of Federal PATH Grant Funds for the continuation of outreach services to the homeless mentally disabled. The application requires an additional non-Federal match ($30,491) which will be provided through existing County Realignment funds. FY 1995-96 is a one-year extension of the original four-year grant. III. REASONS FOR RECOMMENDATIONS/BACKGROUND: Approval of this application will continue the PATH project operated by Phoenix Program, Inc. , under County's Standard Contract #24-385, and will provide additional staffing at the Multi-Service Centers for the Homeless Mentally Ill in San Pablo, Concord and Antioch. The PATH Grant also provides funding for the Outreach Liaison Project, to provide approximately 18 hours per week of case management outreach and support services to the County's homeless mentally disabled population. The Federal PATH Grant Application was reviewed and recommended for approval by the County's Mental Health Commission on July 27, 1995. In order to meet the deadline for submission, the application has been forwarded to the State, but subject to Board approval. Three certified and sealed copies of the Board Order authorizing submission of the application should be returned to the Contracts and Grants Unit for submission to the State.
