RECOMMENDED ACTION: APPROVE and AUTHORIZE the Community Services Director or her designee to execute on behalf of the County, Contract Amendment/Extension Agreement#38-314-1, effective September 28, 1995, to amend and extend Contract #38-314 (FCC/940912) with Sue W. Mead to continue services through December 31, 1995 and to increase the Contract Payment Limit by $9,061, from $23,054 to a new total Payment Limit of $32,115. II. FINANCIAL IMPACT: This contract and the amended payment limit is included in the Community Services Department's approved Head Start Budget for 1995 and is 100% Federally funded. There is no County match. III. REASONS FOR RECOMMENDATIONS/BACKGROUND: In September, 1994 the County Administrator's Office approved and the Purchasing Agent executed a contract with Sue W. Mead for Head Start Family Child Care Demonstration Project services for the period from September 30, 1994 through September 29, 1995. The contract was amended effective August 1, 1995 to expand the contract services to include the duties of the Family Child Care Demonstration Project Coordinator. Contract Amendment/Extension Agreement#38-314-1 extends the contract to continues contractor's services through December 31, 1995.
