SUBJECT: Approval' of Contract Amendment Agreement #26-895-7 with
Jason Appel, M.D.
SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION
I. RECOMMENDED ACTION:
Approve and authorize the Health Services Director, or his designee
(Frank Puglisi, Jr. ) , to execute on behalf of the County, Contract
Amendment Agreement #26-895-7, with Jason Appel, M.D. , effective July
31, 1995, to amend Standard Contract #26-895-6, for the provision of
professional services as Chief of Orthopedic Services. This amendment
increases the payment limit by $60, 000, from $233, 184 to a new total
payment limit of $293,184, and extends the term of the Contract
through October 31, 1995.
II. FINANCIAL IMPACT:
Cost to the County depends upon utilization. As appropriate, patients
and/or third party payors will be billed for services.
III. REASONS FOR RECOMMENDATIONSIBACKGROUND:
On September 27, 1994, the Board of Supervisors approved Contract
#26-895-6 with Jason Appel, M.D. , for the period from August 1, 1994
through July 31, 1995, for professional services as Chief of
Orthopedics at Merrithew Memorial Hospital and Clinics.
Ongoing negotiations are drawing to a close and the purpose of this
Board Order is to provide a mechanism which will assure continuity of
vital services to patients and facilitate payment to the Medical
Specialists for three additional months pending completion of contract
negotiations.
Approval of Contract Amendment Agreement #26-895-7 will allow the
Contractor to provide additional orthopedic services through
October 31, 1995.

AMENDMENT SPECIFICATIONS

In consideration for Contractor's agreement to provide additional
services under the Contract identified herein, County agrees to
increase Contract payment limit. County and Contractor agree,
therefore, to amend said Contract as specified below while all
other parts of the Contract remain in full force and effect:
1. Payment Limit Increase. The Contract Payment Limit set forth
in Standard Contract, Paragraph 4. (Payment Limit) is hereby
increased by $60,000, from $233 , 184 to a new total Payment Limit of
g293,184.
2 . Extension of Term. The term of the Contract specified in
Paragraph 3 . (Term) is hereby extended from July 31, 1995
to October 31, 1995
