Dear Ms . Bishop:
Tt his come to the attention of the Alameda-Contra Costa
Medical Association (ACCMA) that the' Contra Costa County
Board of Supervisors has asked the Contra Costa County Fire
Service to develop recommendations pertaining to upgrading
training to the paramedic level for fire service personnel
serving as first responders in the County EMS system.
The ACCMA appreciates the Board' s interest in enhancing the
quality of service in the Emergency Medical Services (EMS)
system, but believes that consideration of this issue should
take place with input from the medical community and the
Contra Costa County EMS system. There are significant
medical issues involved in consideration of this issue,
including maintenance of skills of paramedic first-
responders, developing protocols for line of medical
authority and authority on the scene, ongoing training and
establishment of quality assurance systems . All of these
considerations require medical input and should be analyzed
in the context of the overall operation of the EMS system.
The Contra Costa County EMS system is currently revising its
EMS Plan to include a new process through which it will
consider enhancements in the EMS system. This process is
intended to include input from all interested parties,
including fire service representatives, EMS administrators
and medical input . Issues pertaining to the role and level
of training of first-responders will be an important
consideration in this process . The ACCMA, therefore, urges
the Contra Costa County Board of Supervisors to refer
consideration of upgrading training first-responder fire
service personnel to the Contra Costa County EMS District .
The ACCMA believes that a collaborative approach to
improving our EMS system will be in the best interests of
patients, EMS participants and Contra Costa County
taxpayers .
