Project of Contra Costa
SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION
I. RECOMMENDED ACTION:
Approve and authorize the Health Services Director, or his designee
(Wendel Brunner, M.D. ) , to execute on behalf of the County, Contract
Amendment Agreement #22-407-6 with the AIDS Project of Contra Costa,
effective April 1, 1995, to increase the payment limit by $17,648 from
$70, 799 to a new Contract payment limit of $88, 447, and to extend the
term of the contract through March 31, 1996.
II. FINANCIAL IMPACT:
This Contract is funded by Federal funds from the Ryan White CARE Act,
Title I, Formula & Supplemental Grants, through intergovernmental
agreements with Alameda County, who is the Grantee of these funds. No
County funds are required.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
on July 26, 1994, the Board of Supervisors approved Standard Contract
#22-407-4 (as amended by Contract Amendment Agreement #22-407-5) with
the AIDS Project of Contra Costa, effective July 1, 1994 through June
30, 1995, to provide HIV/AIDS Case Management service to County
residents diagnosed with HIV Disease and their families.
Approval of Contract Amendment Agreement #22-407-6 will allow the
Contractor to provide additional services, through March 31, 1996.

AMENDMENT SPECIFICATIONS

In consideration of the Contractor's willingness to provide additional
HIV/AIDS case management services as set forth in the Contract identified
herein (and amended by Contract Amendment Agreement #22-407-5) , County and
Contractor agree to amend said Contract as specified below while all other
parts of the Contract remain unchanged and in full force and effect.
1. Extension of Term. The Term of the Contract set forth in Paragraph 3 .
(Term) is hereby extended through March 31, 1996.
2 . Payment Limit Increase. The Payment Limit set forth in Paragraph 4 .
(Payment Limit) is hereby increased by $17, 648 from $70, 799 to a new total
Contract Payment Limit of $88, 447 .
3 . Modification of Payment Provisions. Payment Provisions Paragraph 1.
(Payment Amounts) subparagraph d. , is hereby modified to read as follows:
"[X] d. $ 3,957 monthly for services performed during the
months of July, 1994 through February, 1995;
$ 3,961 for services performed during the month of
March, 1995;
$ 7,337 monthly for services performed during the
months of April, and May, 1995;
$ 7,348 for services performed during the month of
June, 1995;
$ 4, 163 monthly for services performed during the
months of July, 1995 through January, 1996;
$ 833 for services performed during the month of
February, 1996; and
$ 834 for the month of March, 1996; provided that
final contract payment will be withheld from
Contractor pending receipt of a properly
documented final demand and approval by the
Health Services Director, or his designee, of
Contractor's final activity report specified
in Paragraph 3 . (Progress Reports) of the
Service Plan.
4. Submission of Progress Reports. Contractor's Final Activity Report as
specified in Service Plan Paragraph 3 . (Progress Report) shall be submitted
to County's Health Services Director, or his designee, on or before April 15,
1996 .
