RESCIND the Board' s action on August 16, 1994 in appointing Ellzie Ware, Jr. to the Substance Abuse Advisory Board. 2 . APPOINT to the, vacant at-large seat # 4 on the Substance Abuse Advisory Board, replacing Mr. Ellzie Ware, Jr. , to complete an unexpired term ending June 30; 1995 : Paul J. Rich 635 Alhambra Lane Martinez, CA 94553 BACKGROUND: On August 2 , 1994, on the recommendation of Supervisor Smith, Mr. Ellzie Ware, Jr. was appointed to a Supervisorial District II seat on the Substance Abuse Advisory Board. Due to an oversight by staff, on August 16, 1994, Mr. Ware was also appointed to an at- large seat on the- Substance Abuse Advisory Board. Since Mr. Ware had already been appointed to a District II seat, we are recommending that his appointment to the at-large seat be rescinded and that another of the applicants our Committee interviewed, Mr. Paul J. Rich, be appointed to the vacant at-large seat. Project Engineer for environmental water seal of two oil/water treating units in the Shell Oil Refinery. Responsible for all design, funding, procurement and construction activities to Shell. Area Construction Engineer'for construction administration effort on a 600,000 GSF office building/parking deck for the Commonwealth of Virginia. Acted as assistant to Senior Project Manager supervising and coordinating all functions on the project. Responsible for developing and monitoring project reporting procedures including: change control, quality assurance and document control. Conducted preconstruction conferences, progress and payment reviews, contractor requests for changes and time extensions. Prepared formal monthly reports to the owner on progress. Performed constructibility reviews, assisted with development of construction schedules and contracts.
