SUBJECT: Approval of Contract Amendment Agreement #26-236-2 with
Judy Windrix, A.R.T.
SPECIFIC REQUEST(S) OR RECOM.M'ENDATION(S) & BACKGROUND AND JUSTIFICATION
I. RECOMMENDED ACTION:
Approve and authorize the Health Services Director or his designee
(Frank Puglisi) to execute on behalf of the County, Contract Amendment
Agreement #26-236-2, to amend Standard Contract #26-236-1 with Judy
Windrix, A.R.T. , effective August 1, 1994, to increase the two-year
Contract payment limit by $25, 000, from $350, 000 to a new total
payment limit of $375, 000.
II. FINANCIAL IMPACT:
This Contract is included in the Health Services Department Enterprise
I Budget.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
On December 8, 1992, the Board of Supervisors approved Standard
(Novation) Contract #26-236-1 with Judy Windrix, A.R.T. , for the
period from December 1, 1992 through November 30, 1994 , for provision
of medical transcription services for Merrithew Memorial Hospital and
Clinics.
Expert medical transcription services are necessary to meet the
standards for Medical Record Services set forth in the J.C.A.H.
Accreditation Manual and to address the problem of illegibility on the
part of medical professionals for which the Contra Costa Health Plan
has been criticized in State Department of Health Services PHP medical
audits.
Over the two-year term of this Contract, the Hospital and Clinics
staff has required more medical transcription services than were
projected at the time the Contract was negotiated; therefore, the
original two-year Contract Payment Limit is insufficient to compensate
the Contractor for services through the end of the Contract term.
Approval of Contract Amendment Agreement #26-236-2 will allow the
Contractor to continue providing the required services through
November 30, 1994 .

AMENDMENT SPECIFICATIONS

In consideration for Contractor's willingness to provide additional
medical transcriptionist services . under the Contract identified
herein, County agrees to increase the Contract payment limit.
County and Contractor agree therefore to amend said Contract as set
forth below while all other parts of the Contract remain unchanged
and in full force and effect.
1. Payment Limit Increase. The Contract Payment Limit set forth
in Paragraph 4 .. (Payment Limit) is hereby increased by ,525, 000,
from $350, 000 to a new total payment limit of $375, 000.
