THE BOARD OF SUPERVISORS OF CONTRA COSTA COUNTY, CALIFORNIA
DATE: August 16, 1994 MATTER OF RECORD
In its discussion on the subject of improved medical care
transportation and increased hospital choice for East and West
County poor patients and the subsequent action of the Board on
this matter, Supervisor Torlakson requested the Health Services
Director to review annually the number of Basic Adult Care program
patients who would utilize the improved services as noted.

SUBJECT: Improved Medical Care Transportation and Increased Hospital Choice
for East and West County Poor Patients
SPECIFIC REQUEST(S) OR RECOMMENDATION S) & BACKGROUND AND JUSTIFICATION
RECOMMENDATIONS:
I. The County desires to enhance the access to hospital care
for its poor patients who reside in the east and west regions of
Contra Costa County.
Medi-Cal recipients have a choice of health plan or fee for
service hospital care at payment rates established by the State of
California. All hospitals in Contra Costa County accept Medi-Cal
patients, except Doctor's Hospital in Pinole and San Ramon Regional
Medical Center in San Ramon.
The County has a statutory obligation under Welfare and
Institutions Code section 17000 to provide hospital care to persons
who are indigent under the county's standards, and the County
receives state funds to provide hospital care to medically indigent
adults . The County has established the Basic Adult Care program to
provide health care to section 17000 indigents and to medically
indigent adults who, taken together, are the only poor. persons for
whom the County is legally required to provide hospital care.
Basic Adult Care program members are not entitled to Med-Cal
benefits and presently have no choice as to their hospitalization.
They are required to receive their hospital care at Merrithew
Memorial Hospital in Martinez.

In order to promote increased access and choice for the poor,
your staff recommends that the Board of Supervisors adopt the
following measures to improve the ability of all poor persons in
East County and West County to conveniently reach and use the
County's Merrithew Memorial Hospital facilities; 'and to provide
those poor persons in the east and west regions of the County who
are in the Basic Adult Care (BAC) program the ability to choose
local hospital care in the County region in which they live.
FIRST: Increase the number of buses providing shuttle service
between Merrithew Memorial Hospital and the Richmond and Pittsburg
Health Centers from one to four, increase the number of scheduled
shuttle trips from 10 to 42, and provide for evening shuttle
services as shown on Exhibit A.
SECOND: Provide that the shuttle bus service will be
available to all County patients, including all Medi-Cal and BAC
patients.
THIRD: Provide that the shuttle bus service will be available
to friends and family members of patients who may wish to visit
patients or assist in their use of health care services .
FOURTH: Provide that as an alternative to Merrithew Memorial
Hospital, the County will pay for county referred care at one of
the following rates applicable at the time services are rendered,
for the hospitalization of any East or West County Basic Adult Care
(BAC) member who choses an appropriate hospital in their region of
Contra Costa County;
A. The fee-for-service rates for similar services under the
Medi-Cal program. If no final rate has been established for a
hospital for the period and type of services in question, then the
applicable interim rate shall apply.
B. The hospital's Medi-Cal contract rate.
C. The average Medi-Cal statewide'contract rate as reported
annually by the California Medical Assistance Commission.
D. A contract rate negotiated between Contra Costa County
and the hospital.
FIFTH: Contract with Brookside Hospital to provide inpatient
hospital services for West County Basic Adult Care referred care
patients .
SIXTH: Contract with Delta Memorial Hospital to provide
inpatient hospital services for East County Basic Adult Care
referred care patients.
SEVENTH: Provide a public education program for poor
residents of East and West County about the improved transportation
access to Merrithew Memorial Hospital .
EIGHTH: Provide notice through the Contra Costa Health Plan
to all Basic Adult Care members in East and West County about the
choice of hospitals they will have when they are ' referred for
hospital care.
II. Direct the Health Services Director to implement this
program for increased hospital service access for poor residents of
East and West Contra Costa County, and in support of such
, implementation., authorize the following contracts, subject to the
prior written approval of County Counsel;
1 . AUTHORIZE the Health Services Director or his designee to
execute an amendment to the County's existing contract with the
United Council Spanish Speaking Organizations, having a 1994-95
payment limit of $ 240,000 to provide the described increased
transportation services between Merrithew Memorial Hospital and the

Richmond and Pittsburg Health Centers .
2. AUTHORIZE the Health Services Director or his designee to
execute a Contract with Brookside Hospital to provide for inpatient
hospital services for Contra Costa Health Plan and Basic Adult Care
program referred patients, provided that staff or other privileges
accorded county-employed physicians under such contract shall be
limited solely to those privileges necessary for them as county
physicians to provide medical care to Contra Costa Health Plan and
Basic Adult Care program members, at a payment limit not to exceed
$750,000 for fiscal year 1994-95 .
3 . AUTHORIZE the Health Services Director or his designee to
execute a Contract with Delta Memorial Hospital to provide for
inpatient hospital services for Contra Costa Health Plan and Basic
Adult Care program referred patients, provided that staff or other
privileges accorded county-employed physicians under such contract
shall be limited solely to those privileges necessary for them as
county physicians to provide medical care to Contra Costa Health
Plan and Basic Adult Care program members, at a payment limit not
to exceed $500,000 for fiscal year 1994-95 .
4 . AUTHORIZE the Health Services Director or his designee to
enter into a contract with a consulting firm in order to develop
and implement a public education program about the County' s
improved access plan for minority poor county patients . The
contract payment limit shall not exceed $30,000 .
III. It is further recommended that the Board direct that the
implementation of this Order shall not modify medical services
provided by Merrithew Memorial Hospital or the County's outpatient
clinics .

SUBJECT: Approval of Contract Amendment Agreement #26-010-35 with the
United Council of Spanish Speaking Organizations, Inc.
SPECIFIC REQUEST(S) OR RECOMMENDATION(S) at BACKGROUND AND JUSTIFICATION
I. RECOMMENDED ACTION:
Approve and authorize the Health Services Director, or his designee
(Frank Puglisi, Jr. ) , to execute on behalf of the County, Contract
Amendment Agreement #26-010-35, effective August 16, 1994 , to amend
Standard (Novation) Contract #26-010-34 with the United Council of
Spanish Speaking Organizations, Inc. , (effective July 1, 1994 through
June 301 1995) to increase the Contract payment limit by $117, 742,
from $122 , 258 to a new total Contract payment limit of $240, 000, and
to increase the three-month automatic Contract extension payment limit
of $25, 000, from $35, 000 to a new extension period payment limit of
$60, 000.
II . FINANCIAL IMPACT:
This Contract is included in the Health Services Department' s
Enterprise I Budget for Fiscal Year 1994-95.
III. REASONS FOR RECOMMENDATIONSfBACRGROUND:
On June 21, 1994, your Board approved Standard (Novation) Contract
#26-010-34 with U.C.S.S.O. , for patient transportation services
between County Outpatient Clinics in Pittsburg and Richmond and,
Merrithew Memorial Hospital in Martinez; for courier services to pick-
up and deliver laboratory specimens from the outpatient clinics and
from the Richmond Older Adults Clinic to the Hospital laboratory; and
for transportation services for potential users to tour the County's
birthing facilities (PreNatal Patient Tour Program) . This Contract
includes provision for a three-month automatic contract extension
through September 30, 1995.
Approval of Contract Amendment Agreement #26-010-35 will allow for
expansion of the Contractor's fixed route patient transportation
services between County Outpatient Clinics in Pittsburg and Richmond
and Merrithew Memorial Hospital in Martinez.
