SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION
I. RECOMMENDED ACTION:
Approve and authorize the Health Services Director, to execute on
behalf of the County, Contract Amendment Agreement #22-194-6,
effective June 13 , 1994, to amend Short Form Service Contract #22-194-
4 (as amended by Contract Amendment Agreement #22-194-5) with A.
Marice Ashe, to extend the term of the Contract and to increase the
Contract Payment Limit by $20, 000, from $24 , 000 to a new total
Contract Payment Limit of $44, 000, for provision of technical
assistance to the County's Hazardous Materials Commission.
II. FINANCIAL IMPACT:
This Contract is funded by AB 2185 Revenue.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
In March 1994, the County Administrator approved, and the Purchasing
Agent executed, Short Form Service Contract #22-194-4 with A. Marice
Ashe, to provide staffing to the Hazardous Materials Commission on an
interim basis through the end of Fiscal Year 1993-94 , pending
recruitment and hiring of a new permanent Executive Assistant. This
Agreement was subsequently amended by Contract Amendment Agreement
#22-194-5 which allowed the Contractor to provide additional units of
service.
Approval of this Contract Amendment Agreement #22-194-6 will extend
the Contract through August 1994, pending completion of the
recruitment and selection process, and orientation of the permanent
Executive Assistant to the Hazardous Materials Commission.

AMENDMENT SPECIFICATIONS

In consideration for Contractor's agreement to provide additional services
under the Contract identified herein, County agrees to increase the Contract
Payment Limit. County and Contractor agree, therefore, to amend said
Contract as specified below, while all other parts of the Contract remain
unchanged and in full force and effect.
1. Extension of Term. The term of the Contract as set forth in Paragraph
3 . (Term) of Short Form Service Contract #22-194-4 is hereby extended from
June 30, 1994 through August 31, 1994
2 . Payment Limit Increase. The Contract Payment Limit set forth in
Paragraph 1. (Payment Limit Increase) of Contract Amendment Agreement #22-
194-5, is hereby increased by $20, 000 from $24,000 to a new total Payment
Limit of $44, 000.
3 . Increase in Number of Service Units. The number of Service Units set
forth in Paragraph 2 . (Increase in Service Units) of Contract 22-194-5 is
hereby increased by 400 service units, from 480 to a new total of 880 units
of service.
