SUBJECT: Approval of Contract Amendment Agreement #22-431-2 with Bay Area
Addiction Research and Treatment, Inc.
SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION
I. RECOMMENDED ACTION:
Approve and authorize the Chair, Board of Supervisors, to execute on
behalf of the County, Contract Amendment Agreement #22-431-2 with Bay
Area Addiction Research and Treatment, Inc. (BAART) , effective August
1, 1993 , to amend Standard Contract #22-431-1, to increase the payment
limit by $2, 308, from $73, 000, to a new Contract payment limit of
$75, 308.
II. FINANCIAL IMPACT:
This Contract is funded 100% by Federal funds through the Ryan White
CARE Act, Title II, and HUD "Housing Opportunities for People with
AIDS" . No County funds are required.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
On July 13, 1993, the Board of Supervisors approved Standard Contract
#22-431-1 with Bay Area Addiction Research and Treatment, Inc.
(BAART) , to provide Case Management services for Methadone Treatment
Program clients diagnosed with HIV Disease.
Approval of Contract Amendment Agreement #22-431-2 will allow the
Contractor to provide additional services, including, emergency
housing, food and transportation assistance, through June 30, 1994.

AMENDMENT SPECIFICATIONS

In consideration for the Contractor's willingness to provide additional HIV/AIDS Case
Management services under Standard Contract #22-431-1, County and Contractor agree to amend
said Contract as specified below while all other parts of the Contract remain unchanged and
in full force and effect.
1. Payment Limit Increase. The Payment Limit set forth in Paragraph 4. (Payment Limit) is
hereby increased by $2.308, from $73,000 to a new total Contract Payment Limit of $75.308.
2. Modification of Payment Provisions. Payment Provisions Paragraph 1. (Payment Amounts) ,
subparagraph d. , is hereby modified to read as follows:
"[%] d. $ 6.083 for services performed during the month of July, 1993, payable
upon demand, upon approval of the Health Services Director, or
his designee, of Contractor's written data collection reports
as specified in the Work Plan, which is on file in the
County's AIDS Program Office, 597 Center Avenue, Suite 200,
Martinez, and which is incorporated herein by reference;
$ 6.292 monthly for services performed during the months of August,
1993 through May, 1994, payable upon demand, upon approval of
Contractor's monthly written data collection reports, as
specified above; and
$ 6.305 for services performed. during the month of June, 1994;
provided that final payment will be withheld from Contractor
pending receipt of a properly documented final demand and
receipt by the Health Services Director, or his designee, of
Contractor's Final Activity Report specified in Paragraph 3.
(Reports) of the Service Plan."
3. Modification of Service Activities. Paragraph 2. (Service Activities) of the Service
Plan is hereby modified to read as follows:
"2. Service Activities. Contractor shall provide HIV/AIDS Case Management services,
including, but not limited to, emergency housing, food and transportation assistance, for
Methadone Treatment Program clients diagnosed with HIV Disease, in accordance with the Work
Plan which is on file in the County's AIDS Program Office, 597 Center Avenue, Suite 200,
Martinez, and which is incorporated herein by reference."
4. Submission of Reports. Paragraph 3. (Reports) of the Service Plan is hereby modified to
read as follows:
"3. Reports. Contractor shall submit to County's Health Services Director, or his
designee, regular written progress reports, data collection reports and expenditure reports,
in the time, form and manner required by County. Said progress repo is shall include, but
not be limited to, the following:

a. A description of Contractor's progress in conducting said HIV/AIDS case
management services;
b. A cumulative description of the work and services provided;
C. Any difficulties or special problems encountered;
d. Any pertinent facts or interim findings; and
e. A statement that the Contractor is or is not progressing satisfactorily in
conducting and completing the services.
Contractor shall submit on or before August 15, '1994, to County's Health Services Director,
or his designee, a written Final Activity Report which shall include, but not be limited to,
an evaluation of the quantity, quality, and impact of the work undertaken in conducting
services provided under this Contract.
4. Right of Inspection. Upon County's request, Contractor shall make available to
County's AIDS Program Director or her designee for review, all records and materials relevant
to the documentation of services provided under this Contract.
In addition, as requested by County, Contractor shall assist County's designated Contract
Monitor in conducting site visits."
5. Meeting Requirements. Paragraph 4. (Attendance at Meetings) of the Service Plan is
hereby modified to read as follows:
"4. Attendance at Meetings. Contractor shall attend scheduled meetings and training
sessions for Contractors of. the HIV Interagency Network. In addition, Contractor shall
regularly attend scheduled meetings of the Contra Costa HIV/AIDS Consortium."
