John Hutchison, M.D.
SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION
I. RECOMMENDED ACTION:
Approve and authorize the Health Services Director, or his designee,
(Frank Puglisi, Jr. ) to execute on behalf of the County, Contract
Amendment Agreement #26-807-17, to amend Medical Specialist Contract
#26-807-16 (as extended and amended by various Board Orders) , with
John Hutchison, M.D. (Specialty: Orthopedics) , to extend the term of
the Contract from July 31, 1993 through December 31, 1993 .
II. FINANCIAL IMPACT:
Cost to the County depends upon utilization. As appropriate, patients
and/or third party payers will be billed for services.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
For a number of years the County has contracted with Medical and
Dental Specialists to provide specialized professional services which
are not otherwise available in its hospital and clinics.
Under Contract Amendment Agreement #26-807-17, John Hutchison, M.D.
has agreed to provide orthopedic clinic coverage at Merrithew Memorial
Hospital and Clinics through December 31, 1993 , during the unexpected
absence, due to illness, of one of the Department's orthopedists.

AMENDMENT SPECIFICATIONS
Number 26-807-17
In consideration for Contractor's willingness to provide additional
services under the Contract (as extended and amended) identified
herein, County agrees to extend the term of the Contract. County
and Contractor agree therefore to amend said Contract as set forth
below while all other parts of the Contract remain unchanged and in
full force and effect.
1. Extension of Term. The term of the Contract specified in
Paragraph 3 . (Term) (as extended by various orders of the Board of
Supervisors) is hereby extended from July 31, 1993 to December 31,
1993.
2 . Amendment of Payment Provisions. Subparagraphs a. and b. of
Paragraph 1. (Payment) , of the Additional Provisions (as amended by
various orders of the Board of Supervisors) are hereby deleted and
replaced with the following payment provision:
11$63.00 per hour for orthopedic clinic coverage at the
Richmond Health Center. In addition, at the direction of the
Chief of Surgery or the Chief of the Division -of Orthopedics,
Contractor may be assigned to staff orthopedic clinics
elsewhere within the Division of Hospitals and Clinics. "
3 . Revision of Attachment B. Contractor shall provide services
in accordance with the Revised Attachment B which is attached
hereto and incorporated herein by reference.
