SUBJECT: Approval of Contract Amendment Agreement #23-131-3
with Jeri Lynn Bonesteele
SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION
I. RECOMMENDED ACTION:
Approve and authorize the Chair, Board of Supervisors, to execute
on behalf of the County, Contract Amendment Agreement #23-131-3,
effective July 15, 1993 , to amend Standard Contract #23-131-2
(effective August 3, 1992 through August 2, 1993) with Jeri Lynn
Bonesteele, for provision of additional units of service, with a
corresponding increase in the Contract Payment Limit, from
$61, 156 to a new total of $63 , 356.
II. FINANCIAL IMPACT:
Source of funding for this Contract is the IT Corporation Consent
Agreement between IT and the State Department of Health Services.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
On July 28, 1992, the Board of Supervisors approved Standard
Contract #23-131-2 with Jeri Lynn Bonesteele to continue
coordination of the five-year Joint Contra Costa-Solano County
EMS Hazmat Planning Project.
This Project is now drawing to a close, and approval of Contract
Amendment Agreement #23-131-3 will compensate the Contractor for
72 additional hours needed for closure activities related to the
completion of this Project.

AMENDMENT SPECIFICATIONS

In consideration for Contractor's agreement to provide additional
services under the Contract identified herein, County agrees to
increase the Contract Payment Limit. County and Contractor agree,
therefore, to amend said Contract as specified below, while all
other parts of the Contract remain unchanged and in full force and
effect.
1. Payment Limit Increase. The Contract Payment Limit set
forth in Contract Paragraph 4. (Payment Limit) is hereby increased
by $2,200, from $61,156 to a new total Payment Limit of $63,356.
2 . Service Unit Increase. The number of service units set
forth in Payment Provisions Paragraph 1. (Payment Amounts) ,
subparagraph d. (1) is hereby increased by 72 service units from
1,920 to a new total of 1,992 service units.
3. Modification of Service Plan. The Contract Service Plan
is hereby modified by the addition of the following new paragraph:
114 . Completion of the EMS Joint HazMat Project. On or
before the termination date of this Contract, Contractor
shall prepare and submit to the Health Services Director
or his designee, in the form and manner required by
County, a written summary of the five-year IT-funded EMS
Joint HazMat Project; shall distribute any remaining
equipment and supplies to the appropriate county
agencies; and shall close out records and files related
to this Project. "
