Kathryn D. Jung
SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION
I. RECOMMENDED ACTION:
Approve and authorize the Chair, Board of Supervisors, to execute on
behalf of the County, Contract Amendment Agreement #26-250-2,
effective June 1, 1993 , to amend Standard Contract #26-250-1 with
Kathryn D. Jung, to extend the Contract and to increase the contract
payment limit by $24, 904, from $70, 212 to a new total payment limit of
$95, 116.
II. FINANCIAL IMPACT:
Funding for this Contract is included in the Health Services
Department's Enterprise I Budget.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
On October 6, 1992, the Board of Supervisors approved Standard
Contract #26-250-1 with Kathryn D. Jung, for the period from September
1, 1992 through August 31, 1993, for consultation and technical
assistance with regard to Merrithew Memorial Hospital Administrative
issues.
Approval of Contract Amendment Agreement #26-250-2 will extend the
term of this Contract through December 31, 1993 , and increase the
payment limit to provide for payment to the Contractor during the
extension period, from August 31, 1993 through December 31, 1993 .

AMENDMENT SPECIFICATIONS

In consideration for Contractor's availability to provide additional services
under the Contract identified herein, County agrees to extend the term of the
Contract and to increase the contract payment limit. County and Contractor
agree, therefore, to amend said Contract as specified below while all other
parts of the Contract remain unchanged and in full force and effect.
1. Extension of Term. The term of the Contract as set forth in Paragraph
3 . (Term) is hereby extended from August 31. 1993 to December 31, 1993.
2. Payment Limit Increase. The payment limit specified in Paragraph 4.
(Payment Limit) is hereby increased by $24,904, from $70,212 to a new total
payment limit of $95„116.
3. Modification of Payment Provisions. Payment Provisions Paragraph 1.
(Payment Amounts) , subparagraph d. , is hereby modified to read as follows:
" [x] d. (1) $ 5,851.00 monthly for not less than 173 hours of
service per month, or
(2) $ 33.83 per hour for any month in which Contractor
provides less than 173 hours of service; and
(3) County will reimburse Contractor an amount not to exceed
$1,500 for conference registration and travel on behalf
of the County, approved in advance by the Health Services
Director or his designee; for miscellaneous expenses
actually incurred in the provision of services hereunder
during the term of this Contract; and at the rate of 280
per mile for the use of a private automobile required in
the provision of services hereunder. "
3. Insurance Requirements. Special Conditions Paragraph 1. (Deletion of
Insurance Requirements) is hereby deleted and replaced with the following
paragraph.
111. Insurance Modification. Paragraph 19. (Insurance) of the General
Conditions is hereby modified to read as follows:

19. Insurance.
a. Automobile Insurance. During the entire term of this
Contract and any extension or modification thereof, Contractor
shall keep in effect a policy or policies of motor vehicle
liability insurance n2LMing the County and its officers and
employees as additional. insureds, for any use Contractor makes
of a private automobile in the performance of this Contract,
as required by State law and in amounts not less than:
(1) $100, 000 for personal injury to, or death of one person;
(2) $300, 000 for injury to, or death of, two or more persons,
per occurrence; and
(3) $ 50,000 for property damage.
b. Insurance Certificate. Not later than the effective date
of this Contract, Contractor shall provide County with a
certificate(s) of insurance evidencing the above liability
insurance. The policies shall include a provision for thirty
(30) days written notice to County before cancellation or
material change of the above-specified coverage. "
