In such filings, the participating respondents shall be listed. All Respondent Wireline Companies 1. Provide your "gross revenues", as that term is used in Revenue and Tax Code Section 44024, from July 1, 1982, through June 30, 1983. 2. Provide an estimate of your "gross revenues" from June 30, 1Q83 through June 30, 1984, and from July 1, 1Q84, through June 30, 1985. The method- ology and assumptions used in these estimates must be stated and explained. 3• Separate the revenues provided in response to questions 1 and 2 into inter -LATA and intra -LATA toll revenues. Pacific only shall furnish the breakdown of toll revenue to (a) Pacific only and (b) Industry (Pacific plus independents) separated to inter -LATA and intra -LATA. The inter -LATA estimates beyond 1983 shall be projections of pre -divestiture levels of business. 4. Assume that universal service rates are 50% of present rates, and the number of customers is based on a 1983 level of business. Who should be eligible to obtain universal telephone service rates? Suggested Categories: a) Low income - define b) Infirm - define c) Handicapped - define d) Elderly - define e) Other - define 2. Given your suggested category or categories in response to question one, how should the Commission estimate the number of potential applicants for universal telephone service? 3. What administrative method should be used to determine eligibility? What criteria or proofs might be required? Does an individual apply directly to the local service provider? If so, how might the phone company coordinate with the Commission concerning eligibility? 4. What are the differentials in communication needs, costs of providing service caused by geographical differences in the locales of residences, personal income of sub- scribers, and other factors that the Commission should take into account in establishing the rates and character- istics of universal telephone service? How should the Commission weigh these factors? 5. What provisions should a basic minimum service include? Subsidized access only? A measured amount of local calling? A fixed subsidy to be applied both to toll and local calls? 6. With the establishment of universal telephone service rates should the Commission terminate lifeline rates? 7. AB 1348 imposes a surcharge on inter -LATA services. Should the Commission also impose the surcharge on inter -exchange intra -LATA services? Why or why not? 8. Under the very broad definition of telecommunications services in AB 1348, the Commission has been given some discretion in determining categories of services upon which the surcharge will be imposed. What categories of services should be included and why? How can practical distinctions be made? (Please consider video, data, cable services, etc.) 383 9. Given your suggested category or categories in response to question 8, how should the Commission estimate potential revenues? 10. Are there -any service suppliers not listed in Attachment II? Identify. 11. Does the definition of intrastate telecommunication service in Rev. & Tax C. Sec. 44025(a), (b), and (c) exclude any particular service offered by a service supplier? If so, explain. 12. What reports should the Commission use to estimate the gross revenue of each service supplier? a) Responses required by this order (Attachment III). b) Annual reports, if available? c) Tax records? d) Other - describe. 13. In view of the provisions of Rev. & Tax C. Sec. 44184, which permits termination of universal telephone service if there are insufficient funds to pay the bills submitted by the telephone corporations, can the Commis- sion prudently determine a tax rate less than the 4% maximum for the first year? 14. For the eligibility method or methods recommended in No. 1 above, each respondent shall provide an estimate of administrative costs for: (a) starting the program and (b) the annual recurring costs. 15. Provide an estimate of the number of customers who would subscribe to the universal service rate, and an estimate of the dollar support required to provide the service. 2
