SUBJECT:. Approve Submission of Funding Application #28-520 and Notice of
Grant Award #28-520-1 with the Department of Health and Human
Services for the Health Care for the Homeless Project
SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION
I. RECOMMENDED ACTION:

A. Approve submission of an Application for Federal Assistance
#28-520 to the Department of . Health and Human Services in the
amount of $287,738 for the period May 1, 1991 through April 30,
1992 for the Health Care for the Homeless Project.
B. Approve Notice of Grant Award #28-520-1 from the Department of
Health and Human Services in the amount of $85, 097 for six
months of grant support for the Health Care for the Homeless
Project. This Grant Award approves federal funding for the
period from May 1, 1991 through April 30, 1992 . An amended
Notice of Grant Award will be issued by the Department of
Health and Human Services following the Federal Fiscal Year
1992 appropriation to reflect the full-year, base award of
$170, 194 .
II. FINANCIAL IMPACT:
Application #28-520 resulted in Grant Award #28-520-1, providing
$85, 097 of federal funds for six months of grant support for the
Health Care for the Homeless Project. An additional $85, 097 of
federal funds should be received following the Federal Fiscal Year
1992 appropriation, for a total of $170, 194 for the 12-month award.
County's in-kind match of $42 , 657 for the full 12-month period is
currently in the Department' s Budget projection for FY 1991-92.
Funding for subsequent years is anticipated.
III. REASONS FOR RECOMMENDATIONS/BACKGROUND:
Contra Costa County Health Care for the Homeless Project is
federally funded to increase the delivery of primary care services
to the homeless population in the county. The proposal for this
project was originally submitted to the Department of Health and
Human Services by West Contra Costa County Health Care Corporation.
Due to the Chapter 11 reorganization of that corporation, the
Department of Health and Human Services requested that the County
submit application forms shifting the grantee status to Contra
Costa County. To facilitate this change, the application was
signed by the Health Services Director on behalf of the County,
subject to Board approval.

III. REASONS FOR RECOMMENDATIONSLBACKGROUND: (Concluded)
Project goals are: 1) to improve access to primary health care
services for homeless people; 2) to integrate substance abuse
services into the Health Care for the Homeless program and improve
access to substance abuse treatment services; 3) to improve access
to homeless persons to urgent, specialty and inpatient care in
hospitals; 4) to improve access of homeless persons to mental
health, counseling and outpatient/residential programs; and 5) to
assist homeless people who appear to be eligible for entitlement
assistance programs in the completion of applications for such
programs. Services will be provided to the homeless throughout
Contra Costa County at sites in West, Central and East County.
This project is a collaborative effort within Contra Costa County.
The Health Services Department will subcontract with the Public
Housing Authority for operations coordination and client support
services. Other departments with which the Health Services
Department will be coordinating efforts include Community Services
Department (which has donated space for the project office) , Social
Services Department, County Office of Education, and the Veterans
Services Department.
The project is working with the Association of Homeless and Housing
Providers, a consortium of County organizations that will act as an
advisory group to this project, to develop linkages with service
centers, shelters, and housing projects. This project will
establish linkages with other hospitals in the area that may
receive clients on Medi-Cal, including John Muir Hospital and
Brookside. In addition, the Veterans Administration Medical Center
will work with the project on providing services to veterans.
