SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION
I. RECOMMENDED ACTION:
a. Approve and authorize the Health Services Director or his designee
to execute on behalf of the County, Contract Amendment Agreement
#26-158-6, effective March 1, 1990, to amend Standard Contract
#26-158-4, as amended by Contract Amendment Agreement #26-158-5,
with Antonio G. Britto (dba Concord Transportation Services -
Sal 's Taxi of Martinez) , for taxicab transportation services for
Merrithew Memorial . Hospital patients, with a $2, 000 increase in
the contract payment limit; and
b. Approve and authorize the Health Services Director or his designee
to execute on behalf of the County, Novation Contract #26-158-7,
with Antonio G. Britto (dba Concord Transportation Services -
Sal 's Taxi of Martinez), for the period July 11 1990 through June
30, 1991, with a payment limit of $108, 000, for taxicab transpor-
tation services for Merrithew Memorial Hospital patients. This
document includes provision for a six-month automatic extension
through December 31, 1991 with a payment limit of $27, 000 during
the extension period.
II . FINANCIAL IMPACT:
This Contract is included in the Health Services Department's 1989-90
and 1990-91 Fiscal Year Budgets.
The Contractor became the sole provider of taxicab transportation
services during the FY 1989-90 Contract period and was requested by
the Department to increase his services after another provider
canceled his contract. Mr. Britto filled in the gap left by the other
provider, and the payment limit of his FY 1989-90 contract was
inadvertently exceeded by $2 , 000. Approval of Contract Amendment
Agreement #26-158-6 will allow the Contractor to be paid for services
rendered at the request of the Department and will result in a payment
limit increase in the FY 1989-90 payment limit of $2 , 000, from $68, 000
to a new total payment limit of $70, 000.
The Department projects expenditures of $9, 000 monthly during FY 1990-
91 for the Contractor' s taxicab transportation services. The increase
in the payment limit from FY 1989-90 is due to the sole provider
status of the contractor and the increased patient activity which the
Hospital has experienced and expects to continue.

III . REASONS FOR RECOMMENDATIONS/BACKGROUND:
On October 31, 1989 , the Board approved Contract #26-158-4 , and on May
1, 1990, approved Contract Amendment Agreement #26-158-5, with Antonio
G. Britto (dba Concord Transportation Services - Sal 's Taxi of
Martinez) for taxicab services for Merrithew Memorial Hospital
patients for the period July 1, 1989 through June 30, 1990 (with
provision for an automatic three-month extension through September 30,
1990.
Mr. Britto is the sole provider of the taxicab services used by
Merrithew Memorial Hospital to provide transportation for patients to
and from the Hospital and other health facilities, and for transport-
ing Mental Health patients to court, Board and Care homes and recovery
programs.
