SUBJECT: Emergency Ambulance Provider Selection

RECOMMENDATION•
Approve selection of emergency ambulance providers and authorize
Health Services Director to negotiate contracts for Board approval as
follows:
( 1) Regional Ambulance, Inc. , for -Emergency Response ,Areas I,
II, and V as currently served by Regional'; and
(2 ) San Ramon Valley Fire Protection District for Emergency
Response Area IV as currently served by ''San Ramon Valley
Fire.
FISCAL IMPACT:
These contracts will have no General Fund impact. The contract with
Regional Ambulance will involve a subsidy to be paid from service
charges assessed in County Service Area EM-1, Zone 'B.
BACKGROUND:
On June 5, 1990, Your Board authorized the distribution of the Request
for Proposals (RFP) for emergency ambulance service" and directed the
Health Services Director to report back to gYour Board on
recommendations for emergency ambulance providers . ;A copy of the RFP
was distributed to all providers who had expressed interest and to the
California Ambulance Association. Additionally, announcements
regarding the RFP were sent to all ambulance providers licensed in the
State of California. On June 19 , 1990, a bidders , conference was held
to answer questions prospective bidders might have regarding the RFP
or the County's EMS system.

Although active interest had been expressed by a major outside
provider, proposals were submitted only by Regional Ambulance and by
San Ramon Valley Fire for their existing service areas . (Hartson' s
Ambulance of San Diego had expressed an intent to bid and attended the
bidders ' conference. Prior to the proposal due date, however,
Hartson's was selected as the new primary provider in San Mateo County
and notified us that they would not be bidding in Contra Costa . )
Both the Regional and the San Ramon Valley Fire proposals have been
reviewed by Health Services Department staff and by a,; Proposal Review
Panel consisting of persons knowledgeable in EMS from, both within and
outside Contra Costa County. The Report and Recommendations of the
Review Panel (attached) rated both proposals very highly and
recommended selection of these providers .

Report and Recommendations

Background
The Ambulance Proposal Review Panel was appointed ;l'by the Health
Services Department to review proposals submitted in response to
the Request for Proposals (RFP) to provide Emergency Ambulance
Service issued May 21, 1990 . The panel included 15 persons with
expertise in various areas of EMS . Members were selected from both
within and outside Contra Costa County: The following 13 panel
members participated in all deliberations and recommendations :

Leveron Bryant, Asst. City Manager, Richmond
Tom Charron, M.D. , Health Officer, Solano County
Chief Jim Cole, San Leandro Fire, Alameda County
Chief Jerry Fender, Consolidated Fire Protection District
Ellen Folkens, R.N. , Nurse Manager, Kaiser Hospital, Martinez
Sherry Handy, Sheriff ' s Dispatch
Lt. Roy Kinion, California Highway Patrol
Peter Lakes, EMS Director, Marin County
Allen Longshore, M.D. , Consumer, EMCC
Lorraine Olson, R.N. . Base Coordinator, Mt. Diablo Hospital ,
Ted Schreck, C.E.O. , Delta Memorial Hospital
Chief Joe Tovar, Oakley Fire
Two proposals were received in response to the RFP:1'
1 . Regional Ambulance to serve Emergency Response Area (ERA)
I (West and West-Central County) , ERA IIi (East-Central
County) , and ERA V (East County)
2 . San Ramon Valley Fire Protection District to serve ERA
IV (San Ramon Valley)
Both proposals were from existing providers and were for the
geographic areas currently covered by those providers .
Charge
The charge given the Panel was to make recommendations to the
Health Services Department on selection of ambulance, providers and
to make other recommendations it deemed appropriate'iregarding the
specific proposals recommended for selection. The Panel was

directed to base its recommendations on the 1110-minute response
time" option contained within each proposal .
Process
The Panel held two meetings which included interviews with
representatives of each of the proposing agencies. in addition to
copies of the RFP and each of the proposals, the Panel was provided
a staff prepared summary highlighting key features of each
proposal, written material which had been submitted by each of the
proposing agencies in response to questions raised by the Panel
following its first meeting, and copies of questionnaires returned
to the EMS Agency from hospitals and public safety agencies
regarding their recommendations . The Panel was assisted throughout
its deliberations by members of the EMS Agency staff .
Following the Panel's deliberations and interviews with the
prospective ambulance providers, the Panel:
( 1) Determined if each proposal met the minimum standards set
forth in the RFP;
( 2 ) Determined if each . bidder was qualified based upon
experience, resources, and financial structure to provide
the services identified in the RFP;
( 3) Scored each service proposal according to the rating
system set forth in the RFP; and
(4 ) Evaluated the extent to which each proposal provided a
realistic implementation plan and reasonable budget.

Conclusions and Recommendations
The following were the conclusions and recommendations of the
Panel:
( 1 ) Both applicants are extremely well qualified and appear
committed to providing the service requested in the RFP.
(Each applicant received average ratings of over 95 out
of 110 possible points . ) '

( 2 ) Both proposals meet all minimum requirements set forth
in the RFP. !I
( 3 ) The Panel recommends, therefore, selection of Regional
Ambulance and San Ramon Valley Fire Protection to provide
services in accordance with their respective proposals .

(4 ) With respect to the San Ramon Valley Fire proposal,
County staff should require that the FireDistrict make
a clear distinction between the role John Muir Medical
Center staff will play in the District's in':ternal medical
quality assurance program and their ole as base
hospital.
(5) With respect to the Regional Ambulance proposal, County
staff should:
(a) review the components of profit or loss of the
company' s various operations as they -pertain to the
emergency services to be provided in Contra Costa
County, and
(b) monitor and place adequate controls on profits
derived from the new medications restocking and
patient billing program to avoid any windfall in
this area.
