Spooner, Contracts Administrato3@ Costa DATE: August 31, 1989 County SUBJEcT: Approve Contract #29-384 with West Contra Costa Community Health Care Corporation (Preventive Health Services for the Elderly) SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION I. RECOMMENDED ACTION: Approve and authorize the Chairman to execute on behalf of the County, Contract #29-384 with West Contra Costa Community Health Care Corporation in the amount of $41,250 for the period July 1, 1989 through June 30, 1990 for a Preventive Health Services for the Elderly Collaboration Project between the County and the Martin Luther King, Jr. Family Health Center. II. FINANCIAL IMPACT: This contract will provide $41, 250 of federal funding through West Contra Costa Community Health Care Corporation for preventive health services for the elderly in West County. No County funding is required. III. REASONS FOR RECOMMENDATIONS/BACKGROUND: West Contra Costa Community Health Care Corporation has entered into an agreement with the U.S. Department of Health and Human Services to facilitate the access to primary care services by the elderly population of West Contra Costa and to promote linkages between the Martin Luther King, Jr. Family Health Center and the County. This project is supported by the State Preventive Health Care for the Aging Program and Region IX of the U.S. Public Health Service. This contract will pay for a .75 full-time-equivalent Public Health Nurse who will provide community outreach, screening, and follow up services in coordination with the Martin Luther King, Jr. Family Health Center. Contra Costa County Public Health will provide supervision of the Public Health Nurse, administrative coordina- tion, and monitoring of the project in relation to the County's Preventive Health Care for the Aging (PHCA) Program. The goal of the project is to help senior citizens in the Richmond area maintain or improve their health through health problem identifica- tion, referral for problems found, health counseling and education. The Board Chairman should sign three copies of the agreement, two of which should then be returned to the Contracts and Grants Unit ,,QQ for submission to the grantor for signature.
