Approval of Veterans Administration Amendment
SUBJECT: Agreement 4626-023-41
SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION
I . RECOMMENDED ACTION :
Approve and authorize the Chairman to execute on behalf of the
County , Veterans Administration Amendment Agreement 4626-023-41
with the Martinez Veterans Administration Medical Center to
amend Contract 4626-023-37 (effective April 1 , 1988 through
May 31 , 1989 ) , as amended by Amendment Agreements 4626-023-38 ,
4626-023-39 and 4626-023-40 , to increase the payment limit of the
Contract , effective June 1 , 1989 , by an additional $ 100 ,000; to
add new procedures to the list of procedures available under the
Contract ; and to extend the term of the agreement through
December 31 , 1989 .
II . FINANCIAL IMPACT :
This Contract is funded in the Health Services Department ' s
Enterprise I Budget . Approval of Amendment Agreement 4626-023-41
will increase the payment limit of the Contract by $100 ,000 ,
from $100 ,000 to a new total payment limit of $200,000 . There
is no unit price increase included in this amendment .
Cost of the services will depend upon utilization , and the ser-
vices provided under this contract are billable to patients and
third party payors .
; III . REASONS FOR RECOMMENDATIONS/BACKGROUND :
For many years , the County has maintained a contractual rela-
tionship with the Martinez Veterans Administration Medical
Center , through a mutual sharing agreement , which has made
available to the County specialized medical services not
available at Merrithew Memorial Hospital and Clinics due to lack
of resources , equipment , and personnel . At the present time the
Veterans administration is providing nuclear medicine studies ,
specialized laboratory tests , and dermatology services . The
Contract documents have always been prepared by the Veterans
Administration .

On April 26 , 1988 , the Board approved Veterans Administration
Contract #26-023-37 and Veterans Administration Amendment
Agreement #26-023-38 , and on August 16 , 1988 , approved Veterans
Administration Amendment #26-023-399 for the period April 1 ,
1988 through May 31 , 1989 . On July 11 , 1989 , the Board approved
Amendment Agreement #26-023-40 which extended the Contract
through August 31 , 1989 . At that time , both parties understood
that there was to be no unit price increase; however , the
Department and the Veterans Administration inadvertantly
neglected to request an increase in the contract payment limit
to cover the extended term of the Contract .
Approval of Amendment Agreement #26-023-41 will provide for an
increase in the contract payment limit , effective June 1 , 1989 ;
extend the term of the Contract through December 31 , 1989 ; and
add to the list of procedures available to the County, the
following opthalmic procedures :
Visual Field Testing W/Humphrey/Goldman
Vitrectomy
Vitrectomy and Lensectomy
Vitrectomy and Scleral Buckle
Vitrectomy/Scleral Buckle/Lensectomy.
