SS 25210 .4(d) , EM-1 to provide ) 25210 .4a, 25210 . 10, emergency medical ) 25210 . 11(b) , 25210. 13, services ) 25210 . 70, 25210 . 77a, 56653, 56700, 56800, 56842(c) , 56842 .5 & R.&T.C. § 7902 . 7 ) RESOLUTION OF APPLICATION FOR APPROVAL TO INITIATE PROCEEDINGS FOR FORMATION OF COUNTY SERVICE AREA NO. EM-1 The Board of Supervisors of Contra Costa County Resolves that: 1 . Pursuant to the Cortese-Knox Act (Gov.C. §§ 56000 ff . ) , this is a proposal for the formation of a new "EM-1" (Emergency Medical) County Service Area, said formation to be initiated by this Application to the Local Agency Formation Commission for approval of the new service area. 2. The exterior boundaries of the area to be formed into County Service Area No. EM-1 are all of the unincorporated and incorporated territory contained within Contra Costa County. A map showing such boundaries, marked Exhibit "A" , is on file with this Board' s Clerk. 3. The reason for this proposal is to form a county service area to provide emergency medical services (including ambulance, paramedic, trauma, disaster and related training and equipment needed for these services ) on a countywide basis both within and without cities . 4 . The County's Director of Emergency Medical Services shall submit to LAFCO's Executive Officer (together with this Resolution of Application) a service plan for these additional (extended) emergency medical services pursuant to Government Code § 56653 and shall, upon the Executive Officer's request, provide any additional data or information desired pertaining to this matter. 5 . It is also requested that a sphere of influence for the new county service area be established co-terminus with the boundaries of said service area. 6 . The funding for the Service Area is to be from benefit assessment charges obtained from those benefiting from the facilities and services and from user fees in accordance with Gov. C. §§ 25210 . 66a and 25210 . 77a. 7 . No appropriation limit is proposed to be established for this area since it is not presently anticipated that any proceeds of taxes will be expended in support of it. For this reason, pursuant to Gov.C. § 56842, the Board of Supervisors does not propose that this area (upon its establishment) will assume any specific services now funded by the "proceeds of taxes" . Therefore, there are no property tax revenues that are subject to the property tax exchange provisions of Gov.C. § 56842. 8 . This proposal is consistent with the sphere of influence of any affected city or district. 89/571 9 . The Board of Supervisors hereby requests that the Local Agency Formation Commission take action to accomplish the aforestated formation, by proceeding under the Cortese-Knox Act. 10 . The Clerk of this Board is hereby directed to file a certified copy of this Resolution of Application (together with a copy of the said filed map marked Exhibit "A" ) with the Executive Officer of LAFCO.
