HomeMy WebLinkAboutMINUTES - 09201988 - S.2 THE BOARD OF SUPERVISORS OF CONTRA COSTA COUNTY, CALIFORNIA Adopted this Order on September 20, 1988 , by the following vote: AYES: Supervisors Powers, .Fanden, McPeak, Torlakson, Schroder NOES: None ABSENT: None ABSTAIN: None ------------------------------------------------------------------ ------------------------------------------------------------------ SUBJECT: Contra Costa Health Coalition Campaign to Reduce Tobacco Use Supervisor Sunne McPeak presented the attached action plan adopted by the Contra Costa Health Coalition to further the campaign to reduce tobacco use.. Dr. William Walker, County Medical Director, encouraged the Board to approve the action plan. Henry Clarke, General Manager, Contra Costa County Employees Association Local 1 and a member of the Health Coalition, commented on the smoking ordinances adopted by the Board of Super- visors and cities, and urged the Board to approve the additional actions as set forth in the action plan. DeLois M. Basnett, representing the American Lung Association, stated that many stores sell cigarettes to minors and commented on the need for further education programs. She ex- pressed the opinion that Proposition 99 would assist in such education programs. Ms. BAsnett endorsed the Health Coalition' s action plan. Thomas R. Hall, representing the American Heart Associ- ation, related statistics on cardiovascular diseases and noted that smoking is one of the three major risk factors. He urged approval of the Health Coalition action plan. Supervisor McPeak recommended that the Board also reaffirm its support. for Proposition 99 to increase taxes on tobacco products. She also commented on the tobacco industry' s efforts to pre-empt local ordinances through State laws, and noted that the County' s State and Federal legislative delegation should be requested to oppose such efforts.. Supervisor Fanden recommended that in addition to the steps outlined in the action plan, a policy should be developed. that would require the designation of non-smoking rooms in new hotel/motel construction. Board members being in agreement, IT IS ORDERED that the following actions are APPROVED: 1. ENDORSED recommendations of the Contra Costa Health Coalition to further the campaign to reduce tobacco use, as amended to request State and Federal legislative delegation to oppose any effort to pre-empt local ordinances through' State law; 2. ENDORSED Proposition 99 on the November ballot to increase cigarette and tobacco taxes; and 1 3. Further, during one such incident on a Friday afternoon flying from LAX to Oakland, a. PSA flight was cancelled and several people were. forced to another PSA plane. Because of the overwhelming request for non-smoking seats , your personnel declared the entire plane "non-smoking, "which produced a spontaneous,. outburst of cheering from most of the passengers. I was impressed with how strongly everyone felt. I know you and PSA are pioneer entrepreneurs. Perhaps you would also consider pioneering voluntary prohibition of smoking on your planes . BOX 6417 CONCORD, CALIFORNIA 94524 ( 415 )945 - 8720 TO Interested Individuals and Organizations FROM : Sunne Wright McPeak President, Contra Costa Health Coalition DATE : September 9, 1988 RE Recent Actions by the Contra= Costa Health Coalition to Further the Campaign to Reduce Tobacco Use At the September 6, 1988, regular meeting of the Board of Directors of the Contra Costa Health Coalition ( CCHC) , unanimously approved the actions detailed below to further the countywide campaign to reduce tobacco use. The CCHC launched a campaign to reduce tobacco use years ago as a key strategy to both control rising health care costs and improve health status. The CCHC has been represented on the Contra Costa Smoking Education Coalition since its inception by Evelyn Fisher of Brentwood and aggressively supported the smoking regulation ordinances throughout Contra Costa County. Much progress has been made in Contra Costa County to reduce tobacco use, but additional steps are now called for to accomplish our goal of smoke-free communities and a tobacco-free society by the year 2000. The following actions will be pursued by the CCHC as some of the next critical steps in the countywide campaign. a. Request school districts to adopt their own policies to implement the city-county ordinances since the existing ordinances do not cover school districts. School districts collectively are the largest employer in the county, but their employees are not covered by the current laws. The school boards will also be asked to adopt policies and rules prohibiting the free distribution of tobacco products or . sale of tobacco products to minors on their premises. b. Request the county and cities to adopt ordinances banning the free distribution of tobacco products in public places and prohibiting the sale of tobacco Interested. Individuals and Organizations September 9, 1988 Page 2 products to minors (with prominent signing to notify the public of such regulations) . C. Request hospitals to establish and enforce smoke-free environments throughout their facilities (unless specifically.prescribed by a physician for a patient) . Request the. Veterans Administration Hospital . in Martinez to adopt procedures eliminating tobacco use on the premises and banning the distribution of free tobacco products to its patients. d. Request once again all health insurance plans marketing in Contra Costa County to adopt actuarily accurate insurance rates for non-smokers and smokers. Existing research estimates that on the average, non-smokers subsidize the health costs of smokers by as much as $100 per year. The CCHC previously approached all health insurance plans to adopt this approach. While many health plans expressed interest in the concept, only the Contra Costa Health Plan pursued the request. Now that the Contra Costa Health Plan has secured federal approval to implement this practice, it is timely to reactivate the request to all other health plans. e. Request USAir to voluntarily ban smoking on all domestic flights. New federal law bans smoking on domestic flights of two hours or less. Since air in planes is recirculated, smoking anywhere in an aircraft can impact all passengers. One airline has taken the step to ban smoking on all flights in the United States, and it is timely to request USAir as the one air carrier serving Contra Costa County to take a similar leadership role. f. Request the state legislative delegation to oppose any effort to pre-empt local ordinances through state law and secure their endorsement of the above steps. The CCHC will seek their leadership of the Smoking Education Coalition, the American Cancer Society, the American Lung Association, the American Heart Association, the Board of Supervisors, the Mayors' Conference, the Alameda-Contra Costa Medical Association and all other interested organizations and individuals in accomplishing the above action steps. The CCHC is asking all health insurance plans marketing in Contra Costa County to disclose the medical costs of smokers vs . non-smokers and to reflect in their benefit rate structure more accurately the differential costs between smokers and non-smokers. RECOMMENDATIONS in order to promote greater wellness and reduce medical care costs, adopt the following position regarding smoking and tobacco use : a. Endorse the Lmerican Medical Association and Contra Costa Health Coalition goal for a tobacco-free society by the year 2000 and support the following actions : - ban advertising of tobacco products - require stronger health warnings on tobacco products - require labels listing ingredients in cigarette tobacco and other tobacco products - prohibit smoking on any common carrier - desianate smokeless tobacco as a health hazard - eliminate federal price supports for tobacco and increase taxes on tobacco products - support local ordinances to prohibit sale of cigarettes and other tobacco products to minors ; to eliminate the dis' ribution of. free cigarette samples on public property, and to protect the rights of non-smokers. b. Communicate the above position to our legislative delegation requesting support. c. Request the Smoking Education Coalition to develop a community education campaign to achieve the above objectives. d. Direct the Contra Costa Health Plan and request other health insurance plans marketing in Contra Costa to cooperate in an effort to identify the costs for smokers vs. non-smokers, and develop rates that more accurately reflect the true expenditure experience. BACKGROUND The Contra Costa County Board of Supervisors has taken a leadership role in efforts to reduce smoking and tobacco use. The current positions by the American Medical Association and National Academy of Sciences to, among other things , ban all advertising of tobacco and prohibit smoking on airples deserve local support to promote passage. The Contra Costa Health Coalition has adopted the above position. Attached is . a news release which summarizes the position. Also attached are letters which communicate the position to the appropriate parties. The subsidy of smokers by .nor.-smokers in most insurance plans is as much as $100 per year. The Contra Costa Health Plan could develop a marketing strategy for non-smokers and smokers who quit that could be very attractive to prospective members . Contra Costa Health CoaliTion P.O. Coalition DATE: January 6 , 1986 RE Insurance Costs and Rates for Smokers At the December 10 , 1985 meeting of the Contra Costa Health Coalition Executive Committee I introduced the following proposal. ' 1. The Contra Costa Health Coalition request of all health insurers and health plans marketing in Contra Costa a disclosure -of the insurance costs incurred by smokers in comparison to non-smokers. ? . The Contra Costa Health Coalition adoptin concept the position that health insurance and health plan premiums for non-smokers reflect none of the anticipated costs actuarially for smokers. 3 . Refer to _the Contra Costa Health Coalition Health Benefits Design Task Force chaired by Eft-on Hall this issue to .implement and monitor. 4 . Request Evelyn Fisher, the Contra Costa Health Coalition reiDresentative to the Contra Costa Smoking Education Coalition, to present the Contra Costa Health Coalition to the Smoking Education Coalition and seek their endorsement and assistance with the effort. Seek State and Federal legislation (in 1 or more bills ) that will: ( a) reauire all health insurers to file reports with the appropriate State agency disclosing the costs incurred by smokers in their plans versus non-smokers. (b) prohibit insurance premuims for non-smokers to subsidize the health costs of smokers. The State could approach this through all health insurers with Knox-Keene licensure or those providing Medi-Cal services. The Federal government could approach this through all insurers who are HMOs and/or who provide MediCare services. Attached are excellent articles that reaffirm our past positions, including: reduction in smoking (reducing tobacco abuse ) is . the most readily available strategy to both improve health status and reduce medical care costs in the country. although some health insurance plans have incentives for non-smokers , the data reveals that non-smokers still subsidize the medical costs of smokers by perhaps as much as $100 annually. seeking publicly equity and fairness for smokers in in health benefits design makes a powerful statement about the economic drain of smoking through medical care costs and highlights an essential strategy to controlling rising health care costs; this also speaks to consumers where they understand health costs--in their pocketbook.
