Approval of Contract Amendment Agreement #23-071-5
SUBJECT: with Kirby Cooper , Ph.D.
SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION
I . RECOMMENDED ACTION:
Approve and authorize the Chairman to execute on behalf of the
County, Contract Amendment Agreement 423-071-5 with Kirby
Cooper , Ph .D. , effective September 21 , 1988 , to amend Contract
423-071-3 (effective June 25 , 1987 - through September 23 , 1988 ,
as extended by Contract Amendment Agreement 923-071-4) , further
extending said contract through December 23 , 1988 with a payment
limit increase of $9 ,999 , from $53 , 499 to a new total payment
limit of $63 ,498 .
II . FINANCIAL IMPACT :
Source of funding for this contract is the State , under State
Contract 429-321-4 with the State Emergency Medical Services
Authority, and user fees from the participating counties . There
is no increase in the Contractor ' s fee rate , and funds are
budgeted in the Department ' s FY 1988-89 budget .
III . REASONS FOR RECOMMENDATION/BACKGROUND:
On October 1 , 1985 , the Board approved State Contract #29-321
(Standard Agreement EMS-640 ) with the State Emergency Medical
Services Authority for establishment of a Regional Trauma
Registry for Bay Area Counties , with Contra. Costa as the host
County. By separate Board Order , the Department is submitting
for Board approval Contract 429-321-4 with the State in the
amount of $84,,200 for funding of the Bay Area Trauma Registry
Project for the period from June 25 , 1988 through June 24 , 1989 .
Kirby Cooper , Ph.D. , has provided direction , consultation, and
technical assistance to the Regional Trauma Registry Project . for
the past two years under subcontracts which have been approved
by your Board . The Department is requesting approval of
Contract Amendment Agreement 423-071-5 , which will extend the
contract for three months so that Dr . Cooper ' s services may con-
tinue without interruption , pending execution of the State
contract .
