RECOMMENDEDACTION : Approve submission of Grant Application #29-368'i to the The San Francisco Foundation , ' on behalf of the County, requesting $25 ,000 of grant funding for the period September 1 , 1988 - October 30 , 1989 for a new project sponsored by the Public and Environmental Health Advisory Board to establish community-based initiatives to address the prevention of chronic diseases in the community entitled "Communities Organizing for Prevention." II . FINANCIAL IMPACT : Approval of this application by The San Francisco Foundation will result in $25 ,000 of private foundation funding for the Communities Organizing for Prevention Project . Sources of funding are as follows : The San Francisco Foundation $25 ,000 County Budgeted In-Kind Share 11,597 Total Project $36 , 597 III . REASONS FOR RECOMMENDATIONS/BACKGROUND: Chronic diseases are the primary killers of Contra Costa County residents , and are mostly preventable . They account for more than 70% of deaths , and over 30% of years of life lost due to premature mortality. This project will establish community- based initiatives to address the prevention of chronic diseases in the community by addressing smoking prevention and nutrition/ food policies . A community leadership forum and media packages will provide outreach to , and involvement of ,,, schools , local government , businesses , voluntary health associations , and others in prevention activities . This project will implement priority health initiatives designed by the Public and Environmental Health Advisory Board to the Health Services Department . In order to meet the Foundation' s deadline for submission, draft copies of the grant application have already been forwarded to the Foundation , but subject to Board approval .
