Spooner , Contracts Administrator �Jvll` Costa DATE: July 7, 1988 c^' ' SUBJECT: Approval ' of Standard Agreement #29-315-8 ( State k488-94077 ) with. the State Department of Health Services for continuation of the Preventive Health Care for the Aging Program SPECIFIC REQUEST(S) OR RECOMMENDATION(,S) & BACKGROUND AND JUSTIFICATION I . RECOMMENDED ACTION: Approve and authorize the Chairman to execute o'n behalf of the County, Standard Agreement #29-315-8 ( State 4488-;,94077 ) with the. State Department of Health Services in the amount of $50 ,000 for the period July 1 , 1988 - June 30 , 1989 for continuation of the Preventive Health Care for the Aging Program. Ii . FINANCIAL IMPACT: Approval of this agreement by the State will result in '$50 ,000 of State funding for preventive health care for the aging. Sources ;of funding are as follows : $ 50 ,000 State Department of Health -Services „ 58 , 700 County Share in Public 'Health Budget $ 108 ,700 Total Program The County received $46 ,000 of funding from the .State for this program last fiscal year . III . REASONS FOR RECOMMENDATIONS/BACKGROUND : On February 23 , ' 1988 , the Board -authorized submission of a funding application to State Department of Heal',th Services for . continuation of the Preventive Health Care for the Aging Program," also known as the Healthy Older Adult ',,Program (HOAP) . This program provides preventive health education- and . health , promotion activities to the elderly in orderito help senior citizens maintain or improve their health through health problem identification , referral for problems found, health counseling. and, education. Standard Agreement #29-315-8 continues this program during FY 1988-89 . This document has been approved by the Department ' s Contracts ' and Grants Administrator in accordance with the guidelines approved, by the Board ' s Order of December 1 , 11;981 (Guidelines for contract preparation and processing, Health Services Department) . The Board Chairman should sign eight copies of the contract , seven of which should then be returned to the Contracts and Grants Unit for submission to State .
