Approval of Novation Contract 4124-391 -3 with Anita Maxey WU
SPECIFIC REQUEST(S) OR RECOMMENDATION(S) & BACKGROUND AND JUSTIFICATION
I . RECOMMENDED ACTION :

Approve and authorize the Chair to execute on behalf of the
County , Novation Contract #24-391 -3 with Anita Maxey in the
amount of $33 , 483 for the period July 1 , 1987 - February 29 ,
1988 for provision of consultation and technical assistance for
implementation of the County' s Mental Health Supplemental
Service Program (SB 155 ) . This document includes a four-month
automatic extension from February 29 , 1988 through June 30 , 1988
in the amount of $ 15 , 783 .
II . FINANCIAL IMPACT :

This contract is fully funded in the Health Services Department
Budget through the SB 155 Allocation and Supplemental Short-
Doyle funds allocated to the County for fiscal year 1987-88 .
III . REAS-------ONS FOR RECOMMENDATIONS/BACKGROUND :

Short Form Service Contract 4124-391 with Anita Maxey for the
period January 19 , 1987 through June 30 , 1987 was approved by
the County Administrator and executed by the Purchasing Agent.
The contract was subsequently amended by Contract Amendment
Agreements 4124-391 -1 and 4124-391 -2 which included a three-month
automatic extension under which the contractor is currently
providing services . Novation Contract 4124-391 -3 replaces the
three-month automatic extension.
The contract services are vital to the continued development and
implementation of the Supplemental Services Program for Mentally
Disabled Adults as described in the County' s SB 155 Application
submitted on February 5 , 1985 to the State Department of Mental
Health. Under this program, certain licensed residential care
facility operators are provided with training and additional
funding to provide care to more severely mentally disabled
adults . The Department is in the process of creating a position
to replace this Contractor and anticipates that the position
will be in place by the end. of this year. This Contract will be
terminated when the position is in place.
This document has,. been approved by the Department ' s Contracts be
and Grants Administrator in accordance with the guidelines
approved by the Board ' s Order of December 1 , 1981 (Guidelines
for contract preparation and processing , Heal h Services
Department ) . , r,
